2 ATTENTION YOU MAY ONLY REGISTER FOR 1 MOVIE PER MONTH TIME: 3:00 7:30pm COST: $22/per date DATES: Sundays, July 8 or July 29 Sundays, August 5 or August 26 Sundays, September 9 or September 30 We will meet at Logan Center at 3:00p.m. We will view a matinee show at a local cinema, and go for dinner at an area restaurant. The time of our movie will determine if we eat dinner before or after the movie. Movies may be rated G, PG, or PG-13. We do not see rated R movies. Cost will include: your movie ticket, dinner, supervision, and transportation. Please bring extra money if you wish to have snacks at the movies. Please note: a medium Coke & medium Popcorn is $14.10 at the movies! Arrange to be picked up at Logan Center at 7:30p.m. LIMITED TO 15 PERSONS PER DATE YOU MAY ONLY REGISTER FOR 1 MOVIE PER MONTH PLEASE READ CALENDAR DETAILS CAREFULLY! Logan Recreation & Leisure services offers activities in the community and at the Logan Center building for individual s with developmental disabilities ages 18 and above. You must register to attend any of our activities. Please call Dana Molenda or Alicia Hood at Logan Center if you have any questions. Please note: Activity details for our trips will inform you what to expect during our event. Activity details will include what costs will be covered, what you may need to bring with you, and when and/or where to be dropped off & picked up that day. Please keep a copy of our calendar for your records and share activity details with caregivers. Please note, it is important every caregiver knows where an individual is going when coming for Recreation activities to insure individuals are prepared for the days events. Larger Group activities are loosely supervised, and a caregiver will need to attend with you. Activity details will explain what activities are loosely supervised. Recreation events have 1 staff person per 8 individuals attending. When needed, some participants may be asked to have staff stay with them at smaller events to insure they receive enough support during community outings. One-on-one support cannot be provided at all times during events. Remember to always dress according to the weather including a hat, a jacket/coat, Sunscreen, and bug spray if needed. Read calendar details to know when we will be outdoors and dress accordingly. In the event of inclement weather, activities may be canceled or changed. If an activity is canceled or changed, you will be called at the number indicated on your registration form at least one hour before the activity is to begin. You must be registered to get a call! RETURN YOUR COMPLETED REGISTRATION FORM TO LOGAN CENTER REGISTERING FOR ACTIVITIES: Provide your name & phone number at the top of the enclosed Registration Form. List any special needs you have for transportation, assistance, or help during our events. Check the box next to the activity dates you wish to attend. Payment is not required at time of registration. If you send payment, make checks payable to LOGAN Recreation. If an activity is already full & you have sent a payment, your check will be returned with a list of activities you may attend. A REGISTRATION CONFIRMATION FORM will be sent back to you with the activities marked that you are approved to attend. Logan Dances require you to PRE-REGISTER every 3 months when new calendars are received. Tickets will only be sent if payment is included with your Registration Form. If you are pre-registered, you may pay at the door and receipts will be provided. Notifications of changes to dances will only reach those who are pre-registered. Annual Participation Forms are required to attend Any Activity. FRIDAY CHIPPEWA LANES Logan Friday Bowling will BEGIN IN SEPTEMBER at Chippewa Lanes in South Bend. Please see the calendar for specific dates. Friday Bowling will be 4:30 to 6:00pm each week. PLEASE NOTE: Cost will be $3.00 per week to bowl. Cost will include your shoe rental & unlimited bowling from 4:30-6:00p.m. Bowling is a LOOSELY SUPERVISED EVENT! CAREGIVERS and/or STAFF NEED TO STAY with you during this event. NO ONE IS ADDED TO A LANE AFTER 5:00PM! Please update forms as needed and keep information current. All Behavioral, Dining, and/or Risk Plans (i.e. falls, choking, seizures) should be included. ACTIVITY INFORMATION (Complete the enclosed Registration Form) JULY-AUGUST-SEPTEMBER 2018 ACTIVITIES LOGAN CENTER Mondays: July 9, 16, 23, & 30 August 6, 13, 20, & 27 September 10, 17, & 24 NO ACTIIVITY 9/03 Chicago Field Museum of Natural History Saturday, September 29 TIME: 9:00am 8:30pm COST: $80 We will meet at Logan Center at 9:00am. We will travel in the Logan Vans to Chicago, Illinois to tour the Field Museum. We will see one of the Field Museum s newest additions: a Patagotitan Dinosaur; one of the largest Dinosaur skeletons Scientists have ever discovered! We have lunch before going to the Field Museum for our tour and dinner after leaving the Museum. Please eat breakfast before coming to Logan Center we will not eat until we arrive in Chicago! Cost will include your lunch, dinner, Admission to the Field Museum ($38/person), Transportation, tolls, and supervision. Please bring extra money with you to shop in the Museum store or for beverages/snacks at the Museum. Arrange to be picked up at Logan Center at 8:30pm. REGISTRATION REQUIRED LIMITED TO 16 PERSONS TIME: 6:00 8:00p.m. COST: $8/per date We will meet at Logan Center at 6:00p.m. Activities will include: watching a movie, creating holiday crafts, coloring, playing games or working on puzzle books. A beverage and snack will be provided at 7:00p.m. Cost will include snack, beverage, and supervision. We will purchase games and puzzle books for this fun night! Please eat dinner before coming to Logan Center! Arrange to be picked up at Logan Center at 8:00p.m. PRE-REGISTRATION & RELEASE FORMS REQUIRED LOGAN ART CLASSES Thursdays: July 5, 12, 19, 26 August 2, 9, 16, 23, & September 6, 13, 20, 27 NO CLASS AUGUST 30 Time: 4:30 6:00p.m. Cost: $7.50 /WEEK or $90 /SESSION We will meet at Logan Center at 4:30p.m. Classes will begin at 4:45p.m. During Art Classes we will create a variety of art and craft projects with Holiday themes. Classes will be held in the Fine Arts Room at the Logan Center. A pizza party will be held on the last day of class! For those of you at Logan Industries, a ride will be provided from LI to Logan Center at 4:00p.m. each Thursday. Arrange to be picked up at Logan Center at 6:00p.m. Rides from Logan Industries limited to 8 persons and require registration. REGISTRATION REQUIRED LIMIT 16 PERSONS

5 NOTRE DAME FOOTBALL GAME LOGAN CENTER Saturday, September 15 TIME: 2:00 6:30pm COST: $20.00 We will meet at Logan Center at 2:00pm. The Notre Dame vs. Vanderbilt Game is set to begin at 2:30pm. We will have Lunch while watching the game Kick-Off! Lunch will include sub sandwiches, chips, pop, and cookies at Half Time. After lunch, we will watch the Game and Cheer on the Irish! Cost will include your lunch, beverages, and supervision. You can bring additional snacks/drinks with you for later in the game if desired. Please note: we do not have access to the Snack Machines at Logan Center. Arrange to be picked up at Logan Center at 6:30p.m. REGISTRATION REQUIRED LIMITED TO 30 PERSONS COME GET FIT ON FRIDAYS! FITNESS POTAWATOMI PARK DATES: July 6, 13, 20, 27 August 10, 17, 24 NO WALK ON AUGUST 3 TIME: 4:30pm 5:45pm COST: FREE REMEMBER TO BRING A BOTTLE OF WATER! We will meet by the restrooms across from the Pool area at Potawatomi Park in South Bend at 4:30pm. The group will walk around the park at least 3 times; everyone is expected to walk. Please be prepared to walk for at least 45 minutes with minimal breaks in between laps. For those of you at Logan Industries during the day, a Ride will be provided from LI to the Park at 4:00pm. Staff should attend the walk with you, if you have a FALL RISK PLAN. One on One assistance cannot be given to anyone during the walks. Pick will be at Potawatomi Park at 5:45pm. 6 SOUTH BEND CUBS GAME SATURDAY, AUGUST 18 TIME: 11:00am 4:30pm COST: $25.00 We will meet at Logan Center at 11:00am. We will travel to Down Town South Bend to tour the Art Beat festival. Each year, South Bend celebrates performing, visual, and culinary Arts found around our city at Art Beat! We walk about the festival, visit various booths, enjoy live street entertainment, and try some of the food/drinks they have to offer at the festival. Cost will include your Lunch, a $5 souvenir from Art Beat, Transportation, and Supervision. Please bring additional money with you to purchase the Art Work or items for sale at Art Beat. Various booths may have jewelry, home décor, yard items, and/or snacks. We will return to Logan Center after the festival. Arrange to be picked up at Logan Center at 4:30pm. REGISTRATION REQUIRED LIMITED TO 16 PERSONS SUNDAYS: JULY 15 & AUGUST 12 TIME: 1:00 5:00PM COST: $20/PER DATE > FOUR WINDS FIELD/STADIUM < 501 West South St. South Bend, IN We will meet at 1:00pm at Four Winds Stadium on South Street in South Bend. Look for Dana Molenda or Alicia (Lesa) Hood by the old entrance to the Stadium near the flag pole. We will wait out front for everyone to arrive and go into the stadium at 1:15pm. We will attend the South Bend Cubs Game together. Cost will include your ticket to the game and supervision. Please bring additional money with you for concessions, beverages, or souvenirs at the game. Arrange to be picked up at Four Winds Field at 5:00pm. Please make sure we know you are leaving the group at pick up before leaving with staff or care givers! REGISTRATION REQUIRED TICKETS LIMITED TO 15 PER GAME

7 SATURDAYS: JULY 28 OR August 25 TIME: 9:00am 10:00pm COST: $85/PER DATE > > THIS IS NOT A BEACH TRIP! We will visit the Amusement Park only, not the Beach. NO Beach Wear is needed! This trip is for those who enjoy going on Amusement Park rides such as the Scrambler, Flying Swings, Bumper Cars, Tilt-a-Whirl, Antique Autos, & Roller Coasters! Or those who enjoy playing the Amusement Park Games. We will meet at Logan Center at 9:00am. We will travel in the Logan Vans to Monticello, Indiana to visit the Indiana Beach Amusement Park. While at the park, we may enjoy going on rides, playing games, take a half hour boat cruise on Lake Shafer, and walk about the Park. Cost will include your: Transportation, Lunch, Dinner, Admission to the Park, Staff Assistance, and Supervision. You will need to eat Breakfast before coming to Logan. Be Prepared to do a lot of walking and be out in the sun all day! You will need to bring along additional money for souvenirs, beverages/snacks at the park, or to play games. Arrange to be picked up at Logan Center at 10:00pm. REGISTRATION REQUIRED LIMITED TO 16 PERSONS Please note, if storms are forecasted for Lower Indiana on the day we are to travel, this Trip may be re-routed to Michigan s Adventure Park if possible. SAUGATUCK BOAT CRUISE TRIP 2 SATURDAY, AUGUST 11 TIME: 9:00AM 8:00PM COST: $75.00 We will meet at Logan Center at 9:00am. We will travel to Saugatuck, Michigan to tour the town and enjoy shopping in their artisan stores. You will need to bring additional money for shopping in Saugatuck; we can help you with any budget! We will also take a Cruise for an Hour and a Half aboard the Star of Saugatuck Paddle Boat. You will also need to bring along additional money for beverages, candy, or chips during the Boat Cruise. Cost will include your lunch, ticket to the Boat Cruise, dinner, transportation, and supervision. Be prepared to do a lot of walking during this Trip! Arrange to be picked up at Logan Center at 8:00p.m. YOU MAY ALREADY BE PRE-REGISTERED TO ATTEND TRIP 2; PLEAES CHECK WITH DANA MOLENDA IF NOT SURE! REGISTRATION REQUIRED LIMITED TO 8 OPEN SPOTS

8 MINI MEGA PLAY (TUESDAYS > MEGA PLAY) DATES: July 10, 17, 24, & 31 NO ACTIVITY on JULY 3 > THERE ARE NO MINI GOLF DATES IN AUGUST < September 4, 11, 18, & 25 TIME: 4:30 5:45p.m. COST: $12 per date We will meet at Mega Play Family Entertainment Center at 4:30p.m. For those of you at Logan Industries during the day, a ride will be provided from LI to Mega Play at 4:00pm. We will play a round of mini golf, enjoy a beverage, and if time allows, play games. Cost will cover mini golf, a beverage, and supervision. Arrange to be picked up at Mega Play at 5:45p.m. REGISTRATION REQUIRED LIMITED TO 12 PERSONS ***STEPPING UP AND DOWN FROM THE GOLFING GREEN IS REQUIRED DURING THE MINI GOLF COURSE. IF YOU HAVE A FALL RISK PLAN, STAFF MAY BE ASKED TO STAY WITH YOU DURING GOLFING. SATURDAY, SEPTEMBER 22 TIME: 6:00 9:00p.m. COST: $3.00 / ticket Dances are held at Logan Center. Doors will open at 6:00p.m. and dancing/music will begin at 6:30p.m. Rides are not provided to or from the dances. Recreation Rules are to be followed at dances! Do not bring outside music to the dance; DJ will provide the music. You must have an annual participation form on file to attend. Arrange to be picked up at 9:00p.m. DANCES ARE LOOSELY SUPERVISED EVENTS - UP TO 100 PERSONS CAN ATTEND EACH DANCE! STAFF AND/OR CAREGIVERS ARE REQUIRED TO ATTEND THE DANCE & SHOULD MAINTAIN A LINE OF SITE WITH THEIR PARTICIPANTS AT ALL TIMES, INCLUDING WHILE THEY ARE OUTSIDE! A beverage and snack will be provided for all participants. Additional pops will be available for 50 each PLEASE NOTE: > Logan Center has a designated smoking area in the parking lot. Look for the picnic table in the North East Corner towards the back by fence. > Additional Restrooms are always available if needed see Front Desk. > If you see someone behaving inappropriately, becoming upset, or who needs assistance, please report to the front desk Recreation Staff. We will travel to Fort Wayne, Indiana and have lunch before touring the Fort Wayne Children s Zoo. We will have Dinner after leaving the Fort Wayne Children s Zoo. Cost will include your lunch, ticket to the zoo, dinner, transportation, and supervision. You may wish to bring additional money with you for beverages or souvenirs at the zoo. Please be prepared to do a lot of walking during this Trip while at the Fort Wayne Children s Zoo! Arrange to be picked up at Logan Center at 8:00p.m. YOU MAY ALREADY BE PRE-REGISTERED TO ATTEND TRIP 2; PLEAES CHECK WITH DANA MOLENDA IF NOT SURE!
