1 STUDENT APPLICATION PROCESS: TIER 1 STUDENTS A Tier 1 student must be enrlled in a NJ public schl in his r her Resident schl district fr the entire year ( ) immediately preceding enrllment in a Chice district, r if applying fr kindergarten, he/she must have a sibling currently attending the Chice district. Step 1: Cntact the Chice District yu want t attend. Yu will find a descriptin f each Chice District, the seats available, and the telephne number f the district's Parent Infrmatin Center here: Step 2: Cmplete the Ntice f Intent t Participate frm, make tw cpies, and deliver the Ntice f Intent t Participate t yur Sending District in ne f three ways: By Mail: Mail the Ntice f Intent t Participate t yur Sending District via certified mail, return receipt requested. Hand Delivery: Deliver the Ntice f Intent t Participate directly t yur Sending District and request a signed and dated receipt frm the persn t whm yu deliver the Ntice f Intent t Participate; r Chice District Delivery: Deliver the Ntice f Intent t Participate t yur Chice District and they will deliver it t yur Sending District. NOTE: This frm is als available n this web page at Student Applicatin Frms. NOTE: It is essential that yu btain and keep a signed and dated receipt fr the delivery f the Ntice f Intent t Participate in the Interdistrict Public Schl Chice Prgram. The Ntice f Intent t Participate in the Interdistrict Public Schl Chice Prgram must be received by the Sending District by NOVEMBER 2, 2012 What yu shuld get frm the Sending district and when Yur Sending District is required t send yu a Cnfirmatin f Student Eligibility in the Interdistrict Public Schl Chice Prgram prir tnvember 21, 2012, after they have verified that the student is a resident f the district, and that the student is attending the resident district public schl in the current schl year. This indicates that the student is eligible as a Tier 1 public schl student. In the event that the district finds that it cannt verify the infrmatin listed abve fr a particular student, the district must send the Cnfirmatin f Student Eligibility t the parent r guardian f that student, indicating that the student is nt an eligible public schl student fr the Tier 1 applicatin prcess. (Please see prcess belw fr Tier 2 students.) Step 3:

2 Obtain the Applicatin fr Enrllment in a Chice District (frm the desired district). Each Chice District prepares its wn applicatin frm. Cntact each district t which yu are interested in applying and request their applicatin frm. In many cases, yu can find the Chice District applicatins n their websites. Visit the listing f Apprved Chice Districts t find links t Chice District websites. Cmplete the Applicatin fr Enrllment and deliver it t the Chice District. Yu must attach t yur Applicatin fr Enrllment in the Chice District the Cnfirmatin f Student Eligibilty frm yur Sending District that says yur child may participate in the prgram as a Tier 1 student. Make a cpy f yur applicatin and the cnfirmatin fr yur recrds. NOTE: In the event that, after receiving the Ntice f Intent t Participate frm, the Sending District fails t prvide cnfirmatin by Nvember 21 t the student's parent r legal guardian stating whether r nt the student may participate in the schl chice prgram as a Tier 1 student, then the student may apply withut the cnfirmatin frm the Sending District. The Department f Educatin suggests that yu nte directly n yur applicatin that yu have nt received the cnfirmatin frm yur district f residence. T ensure the delivery f the applicatin by the due date, use ne f the fllwing: mail the applicatin t the Chice District via certified mail, return receipt requested; r deliver the applicatin t the Chice District in persn, and btain a signed and dated receipt frm the persn wh receives the applicatin at the chice district. NOTE: It is essential that yu btain and keep a signed and dated receipt fr the delivery f yur cmpleted applicatin. Applicatin fr Enrllment in a Chice District must be received by the Chice District by DECEMBER 3, 2012 What yu shuld get frm the Chice district and when Upn receipt f the Applicatin fr Enrllment in a Chice District, the Chice District must first check whether the applicatin is cmplete. A cmplete applicatin shuld cntain, in additin t the student's name, address and telephne number: the name f the student's Sending District and the schl the student is currently attending; the grade in which the student is enrlled during the current schl year; a cpy f the Individualized Educatin Prgram (IEP) where apprpriate; the Cnfirmatin Frm fr Student Participatin frm the Sending District stating that the student may participate in the schl chice prgram as a Tier 1 student; and

3 any specific infrmatin required by the Chice District in its applicatin. If by December 3 the Chice District receives cmpleted applicatins frm as many r fewer students than the number f seats available in the Chice District fr the fllwing academic year, the Chice District shuld send a Ntice f Cnditinal Acceptance r Rejectin t Parent/Legal Guardian f Student Applicant in respnse t each applicatin received. The Ntice f Cnditinal Acceptance r Rejectin shuld include a Ntice f Intent t Enrll Student in the Chice District fr the parent r legal guardian t cmplete and return t the Chice District. The Ntice f Cnditinal Acceptance r Rejectin must be received by the parent r guardian by December 21. If by December 3 the Chice District receives cmpleted applicatins frm mre students than the number f seats available in the Chice District fr the fllwing academic year, the district must cnduct a lttery fr thse grades r prgrams in which the number f applicatins exceeds the number f seats available. The results f the lttery will determine which students wh have filed cmpleted applicatins will be cnditinally accepted as chice students fr the fllwing schl year, and which students will be placed n the waiting list that must be established by the district. NOTE: Befre cnducting the lttery, the Chice District may give preference t siblings f chice students already enrlled in the Chice District. Each participant in the lttery must be assigned a number reflecting the student's place n the list f accepted students r n the waiting list. Upn cmpletin f the lttery, the district must send the Ntice f Cnditinal Acceptance r Rejectin t the parent r guardian f each student wh is eligible t enrll in the Chice District based n the results f the lttery. The Ntice f Cnditinal Acceptance r Rejectin shuld include the Ntice f Intent t Enrll frm fr the parent r guardian t cmplete and return t the Chice District. The Ntice f Cnditinal Acceptance r Rejectin must be received by the parent r guardianby December 21. Upn cmpletin f the lttery, the Chice District must ntify the parent r guardian f each student wh has been placed n the waiting list in the Chice District based n the results f the lttery. This ntice must include infrmatin abut the waiting list established by the Chice District as a result f the lttery, and must be received by the parent r guardian by December 21. Step 5a: Nte: Parents r guardians wh receive ntice that their child has been placed n the waiting list fr the Chice District shuld skip t Step 5b.

4 Parents r guardians wh receive the Ntice f Cnditinal Acceptance r Rejectin frm the Chice District must decide whether r nt t enrll the student in the Chice District fr the fllwing schl year. If the parent r guardian decides t enrll the student in the Chice District, the parent r guardian shuld cmplete the Ntice f Intent t Enrll. Make a cpy f the Ntice f Intent t Enrll fr yur recrds. The Ntice f Intent t Enrll must be received by the Chice District by January 4, Mail thentice f Intent t Enrll via certified mail, return receipt requested. If yu deliver the Ntice f Intent t Enrll directly t the Chice District, yu shuld btain a signed and dated receipt frm the persn t whm yu deliver it. If the parent r guardian decides nt t enrll the student in the Chice District, the parent r guardian des nt have t submit the Ntice f Intent t Enrll t the Chice District; hwever, the parent/guardian shuld infrm the Chice District as a curtesy. Ntice f Intent t Enrll Student in the Chice District must be received by the Chice District by January 4, 2013 What yu shuld get frm the Chice district and when In the event that, by January 4, the Chice District has received fewer Ntices f Intent t Enrll than the number f seats available in the Chice District fr the fllwing schl year, the Chice District must send Ntices f Cnditinal Acceptance, in the rder assigned thrugh the Chice District's previusly held lttery, t students n the waiting list up t the number f the remaining available seats. The Ntice f Cnditinal Acceptance shuld include the Ntice f Intent t Enrll frm fr the parent r guardian t cmplete and return t the Chice District. The Ntice f Cnditinal Acceptance must be received by the parent r guardian by January 11. Mail thentice f Cnditinal Acceptance via certified mail, return receipt requested. Step 5b: Upn ntificatin by the Chice District r receipt f the Ntice f Cnditinal Acceptance frm the Chice District, the parent r guardian f a student n the waiting list must decide whether r nt t enrll the student in the Chice District fr the fllwing schl year, and must ntify the Chice District as sn as pssible befre January 18. If the parent r guardian decides t enrll the student in the Chice District, the parent r guardian shuld cmplete the Ntice f Intent t Enrll, make a cpy, and deliver it t the Chice District befre JANUARY 18. Parents (Frm First Tier Waiting List) Ntice f Intent t Enrll Student in Chice District must be received by the Chice District BEFORE JANUARY 18, 2013
