1 COUNTY OF ALAMEDA GENERAL AGENCY-TECHNICAL DEPARTMENT ADDENDUM No. 1 to PROJECT No for INSPECTOR OF RECORD (IOR) RFP/SOQ Highland Acute Tower Replacement (ATR) Project DATE: December 7, 2009 Issued by the General Agency, Technical Department, 1401 Lakeside Drive, Oakland, California. TO ALL PROSPECTIVE BIDDERS for the above project, notice is hereby given that the following changes, modifications, corrections, clarifications, and additions, as herein set forth, shall apply to the Request For Proposal/SOQ described herein and shall be made part thereof and subject to all requirements as if originally specified or stated. Receipt of this Addendum No.1 must be acknowledged on the form in the bid proposal in writing. The following Sections of the RFP/SOQ have been modified as follows: Section II, item D. OTHER COUNTY REQUIREMENTS (Page 5) Add the following: 4. Debarment/Suspension Policy: In order to prohibit the procurement of any goods or services ultimately funded by Federal awards from debarred, suspended or otherwise excluded parties, each bidder will be screened at the time of RFP/SOQ response to ensure bidder, its principal and their named subcontractors are not debarred, suspended or otherwise excluded by the United States Government in compliance with the requirements of 7 Code of Federal Regulations (CFR) , 28 CFR 66.35, 29 CFR 97.35, 34 CFR 80.35, 45 CFR and Executive Order The County will verify bidder, its principal and their named subcontractors are not on the Federal debarred, suspended or otherwise excluded list of vendors located at and

Section IV, Item C. COUNTY PROVISIONS, (Pages 18 and 19) Under 1. Small And Emerging ly Owned Business: Revise the definition of a small business (the first paragraph) as shown below: A small business for purposes of this RFP is defined by the United States Small Business Administration as having no more than Four million dollars ($4,000,000) over the last three (3) years.
