Thomas Aquinas Confirmation Retreat. I agree to direct my child to cooperate and conform with the directions and instructions of the parish, school or Diocesan personnel responsible for the activity. I have medical insurance that would cover any hospital, medical and related costs and expenses in the event of illness or accident of an emergency nature. In the event my child is injured or becomes ill and requires emergency medical attention any resulting hospital, medical or related costs and expenses will first be paid by the medical insurance or benefit plan of mine or my spouse. I am not aware of any medical condition of my child which would render it inappropriate for him/her to participate in any such activity. I, hereby give permission to the physician selected by the youth activities supervisory personnel then present to render medical treatment deemed necessary and appropriate by the physician. I hereby grant permission for my child to be photographed and/or videotaped during the event/activity. I understand that my child may decline to be photographed and/or videotaped at any time. I further grant permission for the resulting photographs and/or videotaped footage to be edited, if necessary, and then published and/or broadcast for the purpose of promoting the Life Teen and Edge ministries. Sunday, April 24 th : 1:30pm, Parent Carpools arrive and gather in main hall, listen to some youth share their stories of the weekend! 2:00pm, Carpools depart from Camp Hammer 3:00pm, Arrive back at St. Thomas Aquinas Parish
4 The following list is intended to make your experience an enjoyable one for ALL involved. A PMA! (Positive Mental Attitude) Sleeping bag/blankets Pillow Toiletries (soap, deodorant, toothpaste) Towel(s) Flashlight Water bottle (there are water fountains to keep hydrated.) Sweater/Jacket for cool nights or umbrella (in case of rain) Clothes that can get messy and that you wouldn't miss (i.e. mud football, washable paint) Outdoor shoes Bible, Rosary, Journal, etc. (You ll be glad you brought these) Prescription Medications: These must be turned into Chris by Friday Night! Watch Snack to share with 8 people (fruits, cookies, oatmeal bars, etc) PLEASE DO NOT BRING: Drugs and/or alcohol Valuable jewelry or other valuables. Inappropriate clothing Inappropriate reading material/magazines. Cell phones may be brought but must remain inside suitcases. There is NO CELL PHONE RECEPTION while at camp. Great way to disconnect from the world! YOUR PARENTS WILL PICK YOU UP IF YOU BRING: Or caught using, any type of illegal substance. (i.e. drugs, alcohol, and tobacco). You bring a firearm or knife. You are involved in a major disruption of the Into the Mystery Weekend. Are involved in vandalism of Camp Hammer.

Pizza Dinner 5:30 pm- Youth Depart from OLR/Dermody Parking Lot to Camp Hammer 7:30pm- Teens arrive at Camp Hammer, receive name tags, and unload at cabins. 8:00 pm- Welcome/Opening Session (introduction, rules, small groups) 8:30pm- Session 1: The Mystery Where is God? 10pm- Outdoor Night Game 10:45pm- Students Dismissed Get Ready for bed and lights out SATURDAY: 7am- Core Team Prayer/Meeting/ Glory Stories 7:45am- Optional Morning Prayer 8:00am- Breakfast 9:00am- Session 2: The Mystery Who is God?
