Selective calling service is offered subject to availability of facilities, as a supplement to business main telephone exchange service or to trunk lines. s included in each selective calling area are listed in. 1. Selective calling service is offered to business customers if all lines at the same premises are arranged for selective calling service with the same selected exchanges or localities. B. This service permits customer dialed station-to-station calling at discounted rates to a maximum of ten exchanges, selected by the customer, outside the local service area but within the State of Rhode Island LATA, not exceeding 25 airline miles. Mileage is determined as specified in Section 9. C. Selective calling service is not available on foreign exchange service, or for person-toperson, collect, charge to a calling card, charge to a third telephone number, conference, or other calls which normally require an operator. It is not available with the Business Link plan, the Inward Toll Calling Plan, the Outward Toll Calling Plan, or the Rhode Island statewide calling plan. A. Selective calling service rates are in addition to the rates and charges for the associated main telephone exchange service or trunk lines and associated services. 1. A S&E charge for business customers applies to equip a line for selective calling service in addition to the basic exchange service S&E charge. B. Selective calling service usage rates apply to customer dialed station-to-station sent-paid calls, and to operator completed station-to-station sent-paid calls if facilities are not available for customer dial completion. The rates also apply to operator completed station-to-station sent-paid calls for handicapped persons unable to dial calls because of their handicap. 1. Business Usage For all such calls originating over business exchange lines, selective calling service business usage rates apply. 2. All other calls are charged at the appropriate MTS rates.

9 Page Outward Toll Calling Plan (OTCP) Description A. The OTCP is an optional directly dialed toll discount plan available to business main telephone exchange and/or Centrex service customers. The OTCP is intended to provide rate stability to eligible customers. Customers commit to a minimum amount of MTS usage over 12, 24 or 36 months. Individual customer usage amounts covered by the service agreement are based on the customer's actual usage, but in no case will the commitment be for less than 1,200 hours of annual usage. 1. The OTCP is not available with the Business Link plan, or Selective Calling Service. B. The OTCP allows the customer to pay for MTS usage originating from services through customer communications equipment at a single location and billed to the same account. The effective period for an OTCP may be 12, 24 or 36 months Regulations A. If, during the life of the customer's service agreement, the per minute rates for MTS usage are reduced so that they are less than the OTCP rates, the customer may discontinue the OTCP. B. In addition to the billing call detail associated with the customer's lines, customers receive monthly statements of usage for the account which provide information on the total number of hours used during the current billing period, the total number of hours used to date for the current year, and the total number of hours used to date for the service agreement period. Total hours are factored, as appropriate, to reflect usage by peak, off-peak, and super-peak periods. 1. Usage is calculated on a per second basis. Actual usage is multiplied by the appropriate rate for the time period during which the call was placed. C. Service may be discontinued at any time during the service agreement period. 1. When the customer terminates service prior to the end of the service agreement period, the customer is liable for termination charges equal to the charge for two months of equivalent usage as set forth in the customer's service agreement. 2. When the customer subscribes to an option which provides for charges to be paid in monthly installments, the customer is liable for payment of the monthly installments which are prorated through the date of discontinuance. 3. When the customer subscribes to a payment option which provides for charges to be paid upfront or in monthly installments and the charges for the customer's actual usage exceed the amount billed as of the date of discontinuance, the customer is also liable for payment of charges for usage incurred which exceeds the amount billed as of the date of discontinuance.

If the customer has selected an option which provides for payment of hours of use on an upfront basis, any payment amount which exceeds the charges detailed in this section are credited to any unpaid balance or, in the absence of such balance, are refunded to the customer. D. Relocation When the basic exchange and/or Centrex service associated with the OTCP is relocated to a different premises served by the same or a different central office, existing arrangements for OTCP remain unchanged except for any associated changes in the offpeak, peak and super-peak calling periods. E. With the written permission of the customer and Verizon, the obligation to pay OTCP charges may be assigned to another customer at the same location, provided the business exchange and/or Centrex service associated with the OTCP are also assigned to the other customer. 1. A S&E charge for transfer of service, payable by the new customer, applies for this change. In addition to assuming responsibility to pay the OTCP charges, the new customer assumes the conditions applicable to OTCP service at the time of the transfer. 2. A transfer of service between customers at the same time as a relocation is not permitted. F. Expiration Three months prior to the end of the service agreement, customers may extend their agreement for an additional 12 month period for the usage and payment options applicable to the initial service agreement. 1. Upon expiration of the service agreement, the customer may elect to continue service at the currently effective MTS rates or may subscribe to a new OTCP agreement. G. Timing of completed messages is in one second increments. H. Recontracting for Service 1. Customers may subscribe to a higher commitment amount once during a twelve month period during the life of the service agreement without incurring termination liabilities. 2. The customer may subscribe to lower commitment amounts at any time during the service agreement period provided that charges associated with the current agreement, including any unpaid monthly charges have been paid in full. a. When the customer chooses to lower the commitment amount prior to payment in full of the charges associated with their current agreement, the present value of the unpaid balance of any remaining monthly charges associated with the current agreement must be paid in full prior to the effective date of the new service agreement.

The present value of outstanding monthly charges is determined in accordance with the principles of the time value of money at an effective interest rate specified in the individual service agreement Application of Rates and Charges A. Charges for OTCP consist of an upfront payment and/or monthly charges. 1. The customer may pay the entire charge for the specified usage upfront, in equal monthly installments over the length of the service agreement, or in a combination of upfront and monthly installments. The upfront payment option is calculated based upon the time value of money equivalency factors specified in the individual agreement. B. The customer subscribes to an amount of hours specified in the individual customer service agreement. Unused minutes of the commitment usage amount are subject to rates specified in the customers service agreement and reflect the customer's weighted average of per message and per minute MTS business service rates. Additional minutes used over the number of hours covered by the service agreement are subject to the per minute per message MTS business service rates. Additional minutes of use are accumulated and billed at the end of the month in which they occur. C. OTCP charges apply only to customer dialed station-to-station sent-paid calls and operator completed station-to-station sent-paid calls when facilities are not available for customer dial completion. These provisions also apply to operator completed station-to-station sent-paid calls for handicapped persons unable to dial calls because of their handicap. D. During the OTCP service period the monthly charges are not subject to a Verizon initiated change. However, such rates are under the jurisdiction of the PUC and subject to change upon order of the PUC. E. Service and Equipment Charge applies to establish the OTCP. 1. The S&E charge is the same as that specified in Section 5 to rearrange combined billing. F. Rates and charges are as specified for MTS business, per minute, per message rates. G. Temporary Suspension of Service OTCP charges are not subject to such provisions.
