Purpose: To provide Council with the 2016 Summary Reports for Middlesex Centre s Drinking Water Systems as required under the Safe Drinking Water Act. Background: The Summary Reports were prepared in accordance with Schedule 22 of O. Reg. 170/03 under the Safe Drinking Water Act. We are required to prepare an annual Summary Report for each Drinking Water System and provide these reports to Council not later than March 31 st for the preceding calendar year. Analysis: Enclosed are the 2016 Summary Reports for each of Middlesex Centre s Drinking Water Systems: Birr, Melrose and the Middlesex Centre Distribution System which includes Arva, Ballymote, Delaware, Denfield, Ilderton and Komoka. Financial Implications: N/A Strategic Plans: Middlesex Centre`s Strategic Plan, identified three applicable specific strategic themes as Increasing Customer Satisfaction, Streamline Key Processes, and Environmental Sustainability.

2 January 25, 2017 Page 2 of BIRR (as required by O.Reg. 170/03, Schedule 22) Non-Compliance with Legislations, Regulations, Approvals & Orders During this period, the Facility was operated in full compliance with the SDWA, the regulations and Municipal Drinking Water License, Requirement/Duration of Failure Measures to Correct the Failure Well #1 not being maintained in a manner to prevent entry into well of surface No further actions required, well was water and foreign materials decommissioned. Form 2 not completed for new chlorine pumps Form 2 provided, no further action required O&M Manual not updated to include new well O&M Manual was revised, no further action required The following table outlines the total monthly flows, the average daily flows each month and the maximum daily flows. Comparison of Treated Flow Rates: Birr Well Supply System January February March April May June July August September October November December Average Flow Maximum Flow Rated Capacity N/A N/A 88 %Capacity N/A N/A 34% Birr has a rated capacity of 88 m in the Drinking Water Licence. The maximum day flow occurred in May of This flow was 30.2 m, which is 34% of the rated capacity; therefore the water supplied did not exceed the maximum rated capacity as stated in the Drinking Water License. 3 January 25, 2017 Page 3 of MELROSE (as required by O.Reg. 170/03, Schedule 22) Non-Compliance with Legislations, Regulations, Approvals & Orders During this period, the Facility was operated in full compliance with the SDWA, the regulations and the Municipal Drinking Water Licence, save and except for the following: Requirement/Duration of Failure Measures to Correct the Failure NA NA The following table outlines the total monthly flows, the average daily flows each month and the maximum daily flows. Comparison of Treated Flow Rates: Melrose Well Supply System January February March April May 1, June 1, July 1, August 1, September 1, October November December Average Flow Maximum Flow Rated Capacity N/A N/A %Capacity N/A N/A 39% Melrose has a rated capacity of 277 m in the Drinking Water Licence. The maximum flow occurred in June of This flow was 107 m which is 39% of the rated capacity; therefore the water supplied did not exceed the maximum rated capacity as stated in the Drinking Water License.
