1 Family Vacation Care Authorisation and Booking Form My child/ren has a current Enrolment at Mindarie Keys Outside School Hours Care My child/ren does not have a current Enrolment at Mindarie Keys Outside School Hours Care (an enrolment form must be completed to attend the Vacation Care Program) Children who are not attending Pre-Primary or aged 5 are unable to attend Ecursions. Three Plus offers Vacation Care for these children. Please ensure you read and complete each section of this Form. Emergency Contact Information- Name Relationship to child Contact Number Vacation Care Authorisations I give permission for the above-named child/ren to participate in the ecursions/incursion as I have nominated in the booking section. The children will be supervised at all times by suitably qualified educators who will maintain a minimum ratio of 1 educator to 13 children. Mindarie Keys OSHC aims for a ratio of 1:10. Between 30 and 50 children will attend ecursions. Risk Assessments of each Ecursion and Incursion are available at the service. Please sign each of the following to confirm you understand and agree. There are a limited number of places for each ecursion. My child MUST be booked in advance to attend the ecursion. Fees are taken by direct debit from my nominated bank account or credit card weekly each for ONE week in ADVANCE. Any changes to my child/ren s booked (casual or permanent) days, including cancellations, require a minimum of two weeks notice. In lieu of notice fees will be charged for that time My child/ren must be dressed in accordance with our Sun Protection Policy. This includes having shoulders covered (no singlets) and a sun smart hat. I understand that my child/ren will need to wear sunscreen throughout the day, regardless of the weather. On days when lunch is not supplied by the service I will need to pack a healthy lunch for my child. I understand that the service is an Allergy Aware service and I will not send my child with any nut products (peanut butter, Nutella etc). I will pack a drink bottle filled only with water. Please do not pack lollies, soft drinks etc. Children who do not have a lunch will be supplied one at the cost of $5 which will be added to fees. I give permission for my child to use the bouncy castle and inflatable equipment when available. I give permission for my child/ren to use nail polish, coloured hairspray or chalk, make-up, fake tattoos and face paint. I am aware that the children will travel to ecursions via Service vehicle or hired bus. Children are taken out of the premises on planned ecursions that are based on children s ideas and interests. For ecursions, please have your child/ren at the service by 9.00am to prepare to leave. The bus will return to the service by 00pm, unless otherwise noted in the Ecursion Details.

Children need a packed lunch and morning tea. for 30pm Fremantle Prison & Esplanade Park Ecursion Destination/Address: 1 The Terrace Fremantle and Marine Terrace Fremantle Activities: Children will visit Fremantle Prison and take part in a tour. They will then attend Esplanade Park for a play. Children will need a packed lunch and morning tea. Duration: 6 hrs for 00pm Pizza Party and Slime Making Incursion The children will be provided guidance and supervision as they mi and create slime. Children will need a packed morning and a water bottle. 19th Kwinana Adventure Water Park and BBQ Ecursion Destination/Address: Walgreen Cres, Calista Activities: Children will be able to cool down during water play (not a swimming pool), eplore the Tree Maze or play on the playground. Children need a packed lunch and morning tea. for 30pm 20th Go Karts and Bouncy Castle Incursion What a way to burn off ecess energy! In a safe but incredibly fun day children will enjoy fun and games with their friends. Children will need a packed lunch and morning tea.

3 21st Dec to 4th Jan CLOSED 26/12/18, 27/12/18, 28/12/18, 31/12/18 and 1/1/19 21st 24th 2nd 3rd Eden Beach Sports Day and BBQ Ecursion Destination/Address: Reflection Blvd, Jindalee Activities: Children will enjoy different sports and the playground, followed by a delicious BBQ lunch. Children will need a packed morning tea and water bottle. Duration: 5 hrs for 00pm The Grinch at the Movies, Christmas craft and cake decorating Ecursion Destination/Address: 1244 Marmion Avenue, Currambine Activities: A request from the children who voted for the chosen movie The Grinch. Back at the service they will decorate Christmas cake and create Christmas crafts. Children will need a Duration: 5 hrs Bus departing service 9.00 am returning back at the centre for 130pm Elliott the Littlest Reindeer at the Movies and Nature Play Ecursion Destination/Address: 1244 Marmion Avenue, Currambine Activities: A request from the children who voted for the chosen movie. Back at the service they will eplore the nature playground. Children will need a Duration: 5 hrs Bus departing service 9.00 am returning back at the centre for 130pm Contemporary Art Day Incursion The children will participate in a lot of messy art, including paint balloons, puffy paints and spray paints. Please be aware children will get messy. Children will need a packed lunch, morning tea and a water bottle. 4th Spurs and Sports Ecursion Destination/Address: Spurs Currambine M2/1244 Marmion Ave, Currambine Activities: Children will enjoy lunch at Spurs and a play in their indoor playground. They will then go back to the service for different sports and challenges. Children will need to a packed morning tea and a water bottle. Duration: 3 hrs Departing 100am arrive back at service 30pm

4 7th Jan to 11th Jan 7th Tie Dye and Pizza Party Incursion The children are given the opportunity to create their own Tie Dye T-shirt. Followed by a pizza lunch. Please be aware children will get messy. Children will need a packed morning tea, a water bottle and to supply their own white T-Shirt. 8th 9th Volcano Park BBQ Ecursion Destination/Address: Cnr Abernethy Rd and Wright St, Belmont Activities: BBQ lunch and Play at Volcano Park in Belmont. This amazing park has a massive volcano that has steam rising and slides to go down. Children will need a packed morning tea and a water bottle. for 30pm Water Play Day Incursion Children will enjoy an inflatable water slide and water balloon games. Please pack your child a change of clothes and a towel. Children will need a 10th Jungle Gym Ecursion Destination/Address: 24 Gympie Way, Willetton Activities: The children will enjoy the different climbing frames, obstacle courses and play equipment. Children will need a packed lunch, for 30pm 11th Botanical Golf Ecursion Destination/Address: 25 Drovers Place, Wanneroo Activities: The children will participate in the mini golf eperiences, eplore the gardens and then sit to eat their packed lunch in the shade. Children will need a packed lunch, morning tea and a water bottle. Departing 9am arrive back at service 30pm

5 14th Jan to 18th Jan 14th Kings Park Ecursion Destination/Address: May Drive, Kings Park (Lycopod Playground) Activities: The children will enjoy the play equipment and different sports equipment. They will also bring their lunch for a picnic in the shade. Children will need a packed lunch, morning tea and a water bottle. for 30pm 15th Sumo Suits and Bouncy Castle Incursion Children will take turns to use the Sumo Suits and Bouncy Castle. Children will need a packed lunch, 16th Wild Kidz Ecursion Destination/Address: 7 Lumsden Rd, Wangara Activities: The children will enjoy the different climbing frames, slides and other play equipment. Children will need a packed lunch, morning tea and a water bottle. for 30pm 17th Cuddly Animal Farm Ecursion Destination/Address: 108 John St, Henley Brook Activities: The children will enjoy interactive eperiences with the farm animals as well as bucket and bottle feeding. Children will need a for 30pm 18th Water Play Fun Day Incursion Different water eperiences will be set up including sprinklers and sponge dodgeball. Please pack spare clothing and a towel. Children will need a packed lunch, morning tea and a water bottle.

6 21st Jan to 25th Jan 21st Yanchep Caves Ecursion Destination/Address: Yanchep National Park, Yanchep Beach Rd & Indian Ocean Dr Activities: The children will take a tour of the caves followed by a play on the grassed area. Children will need a packed lunch, morning tea and a water bottle. for 30pm 22nd 23rd 24th Art, Craft and Water Challenges Incursion The children will participate in different art and craft eperiences and different team challenges such as transferring water without spilling any. Please be aware children will get messy. Children will need a packed lunch, morning tea and a water bottle. Aerial World and Dianella Play Space Ecursion Destination/Address: 1/1968 Beach Rd, Malaga and 1/1968 Beach Rd Dianella Activities: Children will enjoy an hour session on the trampolines and then an adventure at the natural play space in Dianella. Children will need a for 30pm Scitech and Planetarium Ecursion Destination/Address: City West Centre, Sutherland St, West Perth Activities: Children will learn about maths and science through play and eperiments then watch a movie about the universe. Children will need a Duration: 4.5 hrs for 30pm 25th Laser Quest and Giant Games Incursion The children will enjoy a game of laser quest with their friends. The giant garden games such as Jenga and Connect Four will be set up for the children all day. Children will need a packed lunch,

7 29th Jan to 1st Feb CLOSED 28/1/19 29th Silent Disco Incursion The children will eperience a fun disco without the added noise by wearing headphones. Other eperiences such as art, nature play, and water play will be set up. Children will need a packed lunch, 30th Braithwaite Park Ecursion Destination/Address: 197 Scarborough Beach Rd, Mount Hawthorn Activities: Braithwaite Park is a natural playground with different water features and challenges. Children will need a packed lunch, for 30pm 31st Games Day Incursion The Games Vault Bus will be attending the service for the ultimate gaming eperience. Children will also be able to play board games between turns in the bus. Children will need a packed lunch, 1st February Ellenbrook Water Park Ecursion Destination/Address: 5519 Library Ave, Ellenbrook Activities: This park offers children fun and challenging play equipment as well as different water play eperiences.
