1 HealthPartners Health (PPO) Primary care clinic (PCC) list This document is a list of the primary care clinics in the HealthPartners Health network. We encourage you to choose a PCC to provide and oversee your medical care. They collaborate with other health professionals and coordinate your care with the appropriate specialists. You don t need a referral from your PCC in order to see a specialist in the network. Use this list to find the name and four-digit clinic number of the PCC you d like to use and enter the information on the enrollment application. You can change your PCC for any reason at any time.
