10 Changes and Revisions to the Standard Standards are revised and updated as needed. This can happen at anytime. Mandatory Five Year Review Members vote on a ballot with the proposed changes/revisions. Members can vote: Affirmative Affirmative with comment Negative (requires a comment) Abstain Abstain with comment Any single negative vote prevents the ballot item from being approved and must be reviewed by the sub-committee and main committee. 23 January

14 Section 4: Owner/Operator s Responsibility 4.1: Each owner/operator shall prepare an Operating Document for each amusement ride or device based on the recommended instructions and specifications provided by the manufacturer. This Operating Document shall be made available to each amusement ride or device operator and attendant. The Operating Document shall include but not be limited to: 23 January

15 Operating Document shall include : Specific amusement ride or device operation policies and procedures Description of the amusement ride or device operation Duties of the specific assigned operator(s) and attendant(s) positions General safety procedures Procedures to follow in the event of unusual conditions or interruption of operation, including an Evacuation Plan Additional recommendations of the owner/operator 23 January

19 Operating Document - Cliffhanger Ride Description: The roller coaster moves the train up the lift hill using a motor driven chain. Once it reaches the top of the crest it operates on gravity. Manufacturer: SDC (Italy) Model: Windstorm Serial #: 28M Date Manuf.: 1994 Height: 49.2 Footprint: x 55.4 Direction of Travel: Counter clockwise Ride Speed: 34 mph Ride time: 1:20 Carriers: 2 trains, 3 cars per train Cycle Time: 2 min Passenger Capacity: 4 ppl/car (12/train) Ride Capacity: 480 pph Environmental Restrictions: LIGHTNING: Clear the ride and cease operations when lightening is in the immediate area. WIND: Do not run the ride in wind speeds of 45 mph. RAIN/SNOW/HAIL: Do not run in heavy snow, rain or hail. Contact MOD in these conditions. Ride Requirements: Height: Min 56 to ride alone, 43 with adult 23 January

20 General Safety Procedures: Ensure all guest areas are clean and free of debris, hazards and tripping/slipping hazards. Do not allow anyone inside fencing while ride is in motion. Authorized personnel only allowed inside operators area during ride operation. Notify maintenance of any unusual sounds or operating conditions. All employees are authorized to shut down the ride if a potentially hazardous situation is observed. Emergency/Evacuation Procedures: FIRE: Notify maintenance and MOD. Unload passengers, close the ride and shut down power. Guide guests to safe area. Attempt to extinguish fire with the appropriate fire extinguisher unless such an effort would be futile or dangerous. At no time should fire-fighting efforts be made if it would jeopardize employee or guest safety. 23 January

21 EMERGENCY/ EVACUATION PROCEDURES: In the event of Emergency press Emergency Stop button! Notify maintenance. Maintenance must evacuate. Inform guests of the evacuation. Assure them that they are safe. Unload ride as quickly and safely as possible. If guests cannot be immediately unloaded, tell them to remain calm. Assure them that they are safe and that they will be attended to shortly. Guide guests to exit gate (and safe/comfortable area if necessary) Notify MOD immediately. POWER OUTAGE: Ride will stop. Unload passengers only if the train is in the load/unload area. Notify maintenance. Maintenance must perform the evacuation. 23 January

22 Incident Procedures: Close the ride immediately. Do not move the guest unless it is necessary for their continued safety. Notify the M.O.D. Use code blue to alert MOD to urgency. Note which persons were involved in the accident. (All persons riding at that time) Retain witnesses and ask them to give a statement to the MOD. Do not re-open until incident site inspection has been performed. Clear this with MOD. Do not apologize or in any way infer blame on yourself or Glenwood Caverns for the accident. Do not provide any medical assistance to any injured parties. Unplanned Downtime If the ride operations are interrupted follow these procedures: Stop the operation and unload the ride. Notify maintenance. Record time in down time log. Remain in position until directed elsewhere by a supervisor. Explain to guests that ride will re-open as soon as possible. Do not be specific concerning the nature of any problem. Unless maintenance has confirmed it as a momentary downtime, do not give guests a time estimate for the re-opening of the ride. Clear the queue if the MOD instructs you to do so. Remain in position to inform guests of the closure and any estimates of its re-opening. Do not leave your position until directed to do so by a supervisor. If the closure was due to mechanical or electrical issues, maintenance must approve the re-opening of the ride. Record the time of re-starting in the down time log. 23 January

23 Pre-Opening Inspection 4.2: The ride or device operator of each amusement ride or device shall conduct a daily pre-opening inspection of each ride or device prior to carrying passengers. This inspection shall include but not limited to: 23 January

24 Pre-Opening Requirements Visual check of all passenger-carrying devices including restraint devices and latches. Visual inspection of entrances, exits, stairways, and ramps. Test of all communication equipment necessary for the operation of the ride or device. Prior to carrying passengers, the ride or device shall be operated for a minimum of one complete cycle. 23 January

25 Training Requirements 4.3 The owner/operator shall provide and document training under the supervision of a trainer for each operator and attendant of an amusement ride or device. This training shall include, but not be limited to the following: Instructions on: Amusement ride or device operating procedures. Specific duties of the assigned operator and attendant position(s) of the amusement ride or device. General safety procedures. Specific procedures to follow in the event of unusual conditions or an interruption of operation. 23 January

26 Training Requirements (cont.) Demonstration by the trainer of the operation of the amusement ride or device. Demonstration by the trainee, under the supervision of the trainer, of the operation of the amusement ride or device. Additional instructions or training under the supervision of a trainer deemed necessary by the owner/operator. 23 January

27 Evacuation Plan 4.4 The owner/operator shall have and maintain an evacuation plan for each amusement ride or device. Standard load/unload area evacuations are the preferred method. Amusement ride or device manufacturer s recommendations. Conditions of the environment that could impact an evacuation. Personnel responsible for performing an evacuation. Notification and cooperation with the outside agencies and entities intended to participate in an evacuation. Identification and location of equipment to support an evacuation. Equipment that may be required to communicate with riders during an evacuation process. Access and egress requirements for personnel and equipment to evacuate the amusement ride or device. Actions required prior to evacuation to prevent inadvertent motion of the amusement ride or device, vehicle(s), carrier(s) and surrounding equipment in the evacuation pathway. Order or sequence of evacuation to efficiently and safely evacuate riders. An appropriate means of egress for evacuees. Removal of riders unable to assist in their own evacuation due to disability, medical condition or other reasons. Procedures for arranging medical assistance as required during an evacuation. 23 January

28 Evacuation Plan (cont.) The owner/operator shall provide and document training on the evacuation plan The owner/ operator shall periodically review the evacuation plan and make adjustments as needed. 23 January

29 Rider Requirements 4.5: The owner/operator.may deny entry to the ride or device to any person, if in the opinion of the owner/operator the entry may cause above normal exposure to risk of discomfort or injury to the person who desires to enter, or if in the opinion of the owner/operator the entry may jeopardize the safety of other patrons or employees. 23 January

30 Rider Requirements (cont.) 4.5.1: Ride or device operators should be given guidelines on the special considerations concerning patron size, and the special considerations applicable to physically disabled and mentally impaired patrons 23 January

31 Signage Requirements 4.6: Signs for instruction to the public shall be prominently placed, bold in design, with wording short, simple, and to the point : Signs for operational instruction or requirements.may be posted at the waiting/loading area or other appropriate location and may include height requirements and other duties and obligations of the passengers 4.6.2: Entrances to machine rooms or restricted areas should be posted when necessary to warn unauthorized persons not to enter. 23 January

32 Patron Responsibility 5.1 There are inherent risks in the participation in or on any amusement ride, device, or attraction. Patrons of an amusement ride, device, or attraction, by participation, accept the risks inherent in such participation of which the ordinary prudent person is or should be aware. Patrons have a duty to exercise good judgment and act in a responsible manner and to obey all oral or written warnings, prior to or during participation. 23 January

33 Patron Responsibility (cont.) 5.2 Patrons have a duty to not participate in or on any amusement ride, device, or attraction when under the influence of drugs or alcohol. 5.3 Patrons have a duty to properly use all ride or device safety equipment provided. 23 January

34 Maintenance Program Requirements 6.1 Based on the manufacturer s recommendations, each owner/operator shall implement a program of maintenance, testing, and inspection providing for the duties and responsibilities necessary in the care of each amusement ride or device. This program of maintenance shall include a checklist to include but not be limited to: 23 January

35 Maintenance Checklist Description of preventive maintenance Description of inspections to be performed Special safety instructions Additional recommendations of the owner/operator 23 January

36 Maintenance Training 6.2 The owner/operator.shall provide training for each person performing the regularly scheduled maintenance on the ride or device, pertaining to their assigned duties. 23 January

37 Maintenance Training Requirements Instruction on inspection and preventive maintenance procedures. Instruction on the specific duties of the assigned position. Instruction on general safety procedures. Demonstration of their duties and inspections. Supervised observation of duties and inspections. Additional instructions deemed necessary by owner/operator. 23 January

38 Pre-Opening Inspection 6.3 Prior to carrying passengers, the owner/operator shall conduct or cause to be conducted a daily documented and signed preopening inspection, based on provided instructions, to ensure the proper operation of the ride or device. 23 January

39 Pre-Opening Inspection Requirements Inspection of all passenger carrying devices including restraint devices and latches. Visual inspection of entrances, exits, stairways, and ramps. Functional test of all communication systems. Inspection or test of all automatic and manual safety devices. Inspection or test of the brakes, including service brakes, emergency brakes, parking brakes and back stops. Visual inspection of all fencing, guarding, and barricades. Visual inspection of the ride structure. The ride or device shall be operated for a minimum of one complete operating cycle. 23 January

41 Unscheduled Cessation Inspection 6.4 An amusement ride or device shall be appropriately inspected and operated, without passengers, to determine that it is functioning properly following an unscheduled cessation of operation caused by: Malfunction or significant adjustment, or Mechanical, electrical, or operational modification, or Environmental conditions that affected the operation, or any combination of the three. 23 January

42 Water Quality 6.5 The owner/operator will maintain appropriate water quality within an amusement ride or device where the guest can reasonably be expected to ingest or contact water volume sufficient to pose a substantial health risk. 23 January

43 Replacement Parts Replacement Parts shall be procured or produced: From original manufacturer Using original manufacturing drawings or specifications Using specifications derived from sufficient analysis to ensure parts of equivalent functions and quality to those provided by original manufacturer and accordance with F January

44 Testing Requirements 6.7 The owner/operator shall be responsible for implementing a program of testing based on the recommendations of the section on Non-Destructive Testing Requirements of Practice F January

45 Inspection Program Requirements Section 7 Program consistent with F770 and F1193 Inspection documents filed in accordance with F770 and F1193 Must promptly notify the manufacturer of an incident, failure, or malfunction which seriously affects the continued proper operation of the ride 23 January

46 Classification of Injuries and Illnesses 8.1 Recording Recommendations: First Aid Log All first-aid treatment including minor injuries and illnesses First-Aid Incident Report Injuries and illnesses that result in hospital admission or where medical treatment is given. (All injuries not classified as minor) 23 January

49 Manufacturer Notification of Injury or Illness 8.3 The owner/operator of an amusement ride or device shall notify the appropriate manufacturer(s) of an incident that resulted in a serious injury within seven days of the occurrence of the incident. 23 January

50 Buying/Selling of Used Rides/Devices Seller shall provide: Buyer with existing manufacturer s documentation Written notice of major modifications the seller has made or caused to be made along with the manufacturer s or other supporting documentation. Purchaser shall: Notify original manufacturer of change in ownership Request from the original manufacturer all updated owner, operational and maintenance manuals and service bulletins 23 January
