3 INSTRUCTIONS TO VISITORS OF CONVENTION EVERYONE ATTENDING CONVENTION MUST PRE REGISTER! Convention attendees who are not voting members of convention are classified as visitors. All visitors must complete a VISITOR REGISTRATION FORM (even if they do not intend to stay at the hotel or register for convention meals). Follow these instructions. SECTION I: REGISTRANT INFORMATION Personal Information: Provide your chapter city, chapter letters, name (as you would like it to appear on your name badge), phone number and address. If you do not have an address, please provide the address of your chapter s tech person. Special Considerations: Please indicate if you have dietary restrictions or require special services. Feel free to call if you would like to discuss your specific need. SECTION II: CONVENTION PARTICIPATION Check ALL designations that describe your role at convention. There may be more than one. Check the extra meetings and gatherings you plan to attend. Please indicate what day you will arrive at convention by the checking the appropriate box. Registration for the convention will be held on the second floor of the Hilton. Greeters and signs will provide directions. Times are listed on the reservation form. To the best of your knowledge, please indicate the registration period you will attend to sign in and pick up your name badge. If you arrive at a time that is not listed, see the convention information desk for assistance. Hotel check-in is located in the street level hotel lobby. It begins at 3:00 pm. You may check in before that time if your room is available. A secure bell closet will be available for your luggage until your room is ready. SECTION III: HOUSING RESERVATIONS IMPORTANT: DO NOT CALL THE HOTEL to make your convention housing reservation. Once you have returned your registration form with fees and your roommate has returned her registration form with fees (if you are requesting a roommate), the convention registrar will make your hotel reservation. Mail your forms early to secure a room in the host hotel. Roommate Preference: Please indicate your roommate preference by checking the appropriate box and listing additional information. Room Rates: Visitors are responsible for their hotel room and any incidentals. EVERY GUEST will be required to provide the hotel with a credit card upon check-in for incidentals. Room fees are listed on your reservation form. They include state and local taxes. Hotel Room Reservation: Convention is Saturday, June 7 through Sunday, June 8, Indicate your check-in date and your check-out date on the form. If you do not wish to reserve a room, indicate that by checking the appropriate box.

4 A note about billing: Even though you are pre-paying your housing expense, the hotel will require that each P.E.O. check in at the hotel desk to provide a credit card number. This card will be used for incidentals (room service, etc). SECTION IV: MEAL RESERVATIONS Meal reservations are required for convention meals. Please indicate your reservation preferences by checking the appropriate box(es) and list the fee(s) accordingly. Note- Non-P.E.O. guests are welcome to attend the Projects Dinner and Program on Saturday, June 7. If you would like to invite a non-p.e.o. guest or guests to attend the Projects Dinner, add the appropriate fees per guest. Please list the name(s) of your guest(s) on the registration form. You may pick up your guests meal ticket(s) when you sign in at the registration table and obtain your convention badge. If you are arriving on Friday, there is a restaurant in the hotel where you can get breakfast on Saturday morning. Or, if you prefer, the hotel provides a shuttle service to the Mall of America. SECTION V: REGISTRATION FEE According to the Minnesota State Bylaws, visitors attending convention shall pay a fee of five dollars ($5). This fee will be waived for those who are attending only the Projects Dinner and/or Project Program. SECTION VI: PAYMENT Add all fees together. Make checks payable to: MN State Chapter, P.E.O. Reservations MUST be received by: Wednesday, April 23, 2014 IMPORTANT! For a variety of reasons, a sister will make a reservation that she is unable to keep. If this describes you, PLEASE offer your reservation to another P.E.O. and provide Susan DeFranco with your replacement s name. Due to the conditions of the contracts negotiated between the P.E.O. Sisterhood and the Hilton Hotel, changes or refunds cannot be made following the registration deadline. Thank you for your understanding. 5 Please Note: Chapter Delegate: please use the yellow Member of Convention Registration Form. Hotel reservations will be made in the order they are received. Both your roommate s registration form (if you are requesting a roommate) and your registration form, including all fees must be received before a hotel reservation can be made. If possible, mail them in the same envelope. To ensure a room at the host hotel, please mail your forms early. An updated convention schedule will be posted at in April. Be sure to check for that and other convention information on the web.
