These must be sent ASAP to bfroese@telus.net You must also scan and the attached PHOTO PERMISSION SLIP (page 13 ) to bfroese@telus.net TEAM INFORMATION: Please forward a complete HOCKEY CANADA ROSTER (up to 19 players & 5 team officials) to bfroese@telus.net ASAP. We will also need your home & away jersey colors. Please send your accommodation information as soon as you have booked. Please forward any MEDICAL INFORMATION you have on file regarding players so we may be prepared to provide the best service in case of an emergency. BANQUET INFORMATION The 2012 BC Hockey Midget Tier 3 Provincial Championship Banquet is an event that all teams MUST attend. We have a healthy meal planned, as well as a guest speaker(s). The banquet will be for players, team officials, BC Hockey representatives, on ice officials and dignitaries only. Please inform us if there are any dietary issues, such as allergies. Banquet: 6:00 pm (doors open at 5:30 pm) Location: Sonora Centre, th Ave, Osoyoos Dress: Team Track Suits, or Dress Attire. Date: Sunday, March 18, 2012

5 COACHES/MANAGER MEETING There is a MANDATORY Coaches and Managers meeting immediately following the banquet at the Sonora Centre. Meeting room will be announced at the banquet. - Introductions - Review of the Rules & Regulations (BC Hockey Representative & RIC) - Collection of Team Registration Materials - Distribution of Tournament Packages (Chairperson) - Information & Question Period TOURNAMENT PASSES Tournament passes can be ordered in advance at a cost of $15.00 for an individual week pass, and $20.00 for a family (2 adults and up to 4 kids [under 13 yrs] ) week pass. Parents are encouraged to pre-order tournament passes which will be distributed to the team manager at the coaches meeting prior to your team s first game. Pricing at the door will be $2.00 per game or $10.00 for a day. Players and all accredited coaching staff will be provided with a tournament pass. These will be REQUIRED in order to gain entry to the arena facilities during the Championships. Please remember, there are no tournament fees that are paid to the tournament host association to enable participation in the Championship; therefore costs will be offset by entry fees to the arena facilities. FILL OUT THE ATTACHED ORDER FORM (page 11 ) and to bfroese@telus.net. Payment amount may be added to the overall team invoice if desired, and MUST be received prior to the tournament. OPENING CEREMONIES Opening Ceremonies will take place on Monday, March 19, 2012 at 7:30 pm at the Sun Bowl Arena. TEAM ATTENDANCE IS MANDATORY. The two teams participating in the game immediately following the ceremonies should be dressed in full equipment. The remaining teams are asked to wear appropriate team attire and their HOME jerseys. All teams are to be at the arena no later than 7:00 pm. If you have a team banner you would like to carry please feel free to bring it with you.

We suggest that when you book your rooms, please mention it is for the BC Championships. We encourage players, parents and team staff to view the items as soon as your team has qualified to participate, and order in advance (order form page 12 ) as there will be a very limited supply on hand for sale.
