1 1 Dear Parent/Guardian, Thank you for choosing Camp Fit & Fun as your child s summer destination spot! This packet is to provide you with pertinent information about camp this week. We would like to request that your camper bring a bagged lunch, a water bottle (with name on it), and two snacks daily. We also strongly recommend sunscreen and a hat everyday! Camp begins at 8:00 am and will end at 4:30 pm daily. On Tuesday we invite campers to bring in bikes, roller blades, and skateboards. Appropriate safety equipment (helmet) is required for your child to participate in this activity. Wednesday is bring a friend day. Don t forget to bring in Super Soakers and bathing suits on Thursdays! At the end of each camp day campers will have two movie choices. We will have the movie title choices available to parents each morning for approval. Once again the staff of Camp Fit & Fun would like to thank you for choosing us! 2 2 Dropping Off/Picking up Procedures Dropping Off Procedure: Camp begins at 8:00 a.m., please do not drop off your child prior to this starting time or you will be charged for pre-care. Each morning of camp please escort your child/children to The Renaissance Academy lower school lobby area (car rider line drop off). Please see the Welcome to 2016 Summer Camp! that was sent to you for more drop off instructions. Prior to leaving, sign in your child with one of the Camp Fit & Fun staff. Picking Up Procedure: Campers are expected to be picked up no later than 4:30 p.m. Please sign out your child with one of the Camp Fit & Fun staff. Staff members will only release campers to adults with photo ID and on the official pick up list. Campers who are not picked up on time will be charged $10 (per child) for every 15 minutes of lateness. The staff greatly appreciates you following these procedures in order for the camp to run safely and smoothly. Thank you. 4 4 Dear Parent or Guardian, This form serves as permission for your child to participate in a few things we do at camp. Please check the items that you give your permission to participate and sign and date the bottom. Rock Wall Initial This is supervised by 2 or more teachers at all times. Movies Initial There are age appropriate movies show while campers wait to be picked up. Swimming Initial There CFF takes all campers to spring city pool club. There are 3 pools for those that swim, and those that are too young. Policies and Such Initial Please initial here stating that you have read and understand our Policies and Such (found on the last page and Pictures and Videos If you DO NOT want your child s pictures or video to appear in camp newsletters, camp website, DVDs, etc, for advertising purposes, SEE A STAFF MEMBER ASAP I Give my permission for all the above that are checked/initialed by me. Students need a towel and a bathing suit with dry clothes to change into. Campers will be swimming for a few hours.

5 Policies and Such CANCELLATIONS - Families that cancel after paying the required $50 deposit will not be refunded. If additional payments were made, all but the $50 will be gladly refunded. If a camper is pulled anytime after the first day of camp, only HALF of the total payment will be refunded. LATE PICK UP POLICY - Families that are late to pick up campers will agree to pay $10 for every 15 minutes late. CONDITIONS OF ENROLLMENT -To provide a safe and optional camp experience, accommodations that are offered in the school may be provided. Outside Therapeutic Support Staff are strongly recommended to continue during the camp sessions to help with children s needs. - Camp Fit & Fun strives to provide a positive camp experiences for children with various needs whenever possible. However, Camp Fit & Fun does not provide programs that are rehabilitative or therapeutic in nature, and does not specialize in serving children with special needs, including children with emotional, social, or behavioral difficulties. Please provide information (attach a separate sheet) regarding your child s special needs, including any disabilities, disorders or medical problems with this application form. To provide a safe and optional camp experience, accommodations that are offered in the school will need to be provided. Outside Therapeutic Support Staff necessary to continue during the camp sessions to help with children s needs. RATIONAL FOR HOURS Of OPERATION - Our camp day is scheduled to exceed average school day hours. For those students who attend The Renaissance Academy, our hours of operation are longer than normal school hours, with pre-care we are open 50 minutes earlier then the start of school and close 30 minutes past school closing time. We have made a sincere effort to accommodate our working parents schedules. We have decided for this reason to cancel our pre and post care program, also a lack of participation in the pre and after care program with a lack of staffing options also contributed to this decision. Our thinking is that parents and care givers are used to drop off and pick up times for the majority of the calendar year so our hours of operation in exceeding these hours should more than meet the needs of the population we serve. Camp Fit & Fun is not responsible for articles of clothing or personal belongings/cash said to be lost for any reason.
