1 The Salvation Army Camp Walter Johnson Parent Information p. 1 of 3 To: Parents of children attending The Salvation Army Camp Walter Johnson 2015 From: Major Charles Whiten, Camp Walter Johnson Director & Divisional Youth Secretary for The Salvation Army North and South Carolina Division We have been planning a summer of fun and excitement for the children who will be attending Camp Walter Johnson. Camp will be a great opportunity for your child to make new friends, explore God s beautiful world in nature, discover the boundless love of Jesus Christ, and have tons of fun while doing it! Please read over this information carefully, as it will help you and your child plan and prepare for a great experience! FORMS AND ATTACHMENTS With this letter, you will find the following forms and attachments. Please fill them out and return them to your local Salvation Army camp representative by (date). Camp Check-Off & Packing List (for your use; do not turn in) Permission/Waiver for Participation at Salvation Army Residential Camps Health History Form Challenge Course Permission Form (for campers ages 10 & up) Free/Reduced Meal Application Medical Statement for Special Nutritional Needs (only needed for campers who need special dietary modifications) o Part A to be completed by Parent/guardian o Part B to be completed by Licensed Physician Parental Request form for Fluid Milk Substitution (only needed for campers who cannot drink milk) SALVATION ARMY CONTACT INFORMATION If you have questions regarding camp attendance, please contact your local Salvation Army directly. Eligibility, camp forms, transportation arrangements, registration fees and other information are determined locally. Your camp contact person will be able to answer your questions or he/she will get in touch with the camp administration directly if needed. TRANSPORTATION Transportation arrangements to and from Camp Walter Johnson will be determined by your local Salvation Army. Please contact your local Salvation Army camp contact person to obtain information such as where/when your child should report for camp, where/when your child will return home, etc. CAMP ADDRESS - CAMP WALTER JOHNSON; 918 Walter Johnson Road; Denton, NC Campers love to get mail: we announce their names during mail call each day, and the children are eagerly listening for their names to be called. This really helps keep down homesickness for those who receive mail. We suggest sending your letter early in during your child s camp session or even a day or two before he leaves for camp to make certain your letter arrives while your child is still at camp. Although we have provided the camp address for you, it is for the safety of the children that we cannot have unexpected visitors at camp. All adults at camp must be staff employees or volunteers who have prior approval and training to participate.

2 The Salvation Army Camp Walter Johnson Parent Information p. 2 of 3 COMMUNICATION WITH CAMPERS FROM HOME Campers need to focus on their camp experience, and phone calls from parents tend to create home-sickness for the child as well as difficulties for their cabin-mates who suddenly want to talk with their own parents, too. While your child is at camp, he/she will not be allowed to use the camp phone, nor the cell phone of camp staff or leaders. Please consider including stationery and stamps for your child to write home. Be assured that your child will be well-cared for while he/she is at camp, but if a situation arises that warrants communication with a parent, the camp director or other camp administrator will contact the parent directly. For the reasons stated above, and for a number of security and privacy reasons, campers may not bring cell phones to camp. The only exception to this is for those who will be participating in the five-week Music and Arts Conservatory. Separate guidelines on cell phone use privileges will be sent separately for these campers. IMPORTANT NOTE ABOUT MEDICINE, EPI-PENS If your child will take medicine at camp, information as to what medicine is to be taken, as well as frequency and dosage must be indicated on page 1 of the HEALTH HISTORY FORM. Also, the prescription medicine MUST have your child s name on the label and match the dosing amount and frequency indicated on the form. If child becomes sick prior to camp and is prescribed an antibiotic or other medication to be given during camp, please ADD this information to the camp form, OR write a note to be included with the camper health form. If you have more than one child attending camp taking the same medicine, each child must have his/her own medicine. The dosage and frequency instructions for the child must be written separately on their own forms. If you have a child who needs to take non-prescription (over-the-counter) medicine while at camp, this also must be indicated with medicine/dosage & frequency on the child s health history form. Please pack all medications for each child in ONE large zip lock bag, labeled with your child s name and corps or service center information in permanent marker. If medicine must be kept cool, please provide a cool pack and insulated case for transport to and from camp. CAMPER BEHAVIOR Please discuss the importance of good camper behavior with your child prior to camp. Camp Walter Johnson has a caring and capable staff, and your child should understand that proper respect and cooperation needs to be shown to their counselors and other teachers, as well. Also, respect and kindness shown to other children in their cabin and groups will go a long way in helping your child have a great camp experience in making new friends that they will look forward to seeing again summer after summer at camp. HEAD-LICE & NIT POLICY FOR CAMP WALTER JOHNSON No child will be allowed to attend camp if head lice or nits are present even within the week prior to camp. This is to ensure a minimal risk of re-infestation during the week at camp. In the month prior to camp, your child s head will be checked periodically during Salvation Army youth programs, in order to catch any problems in time to get them cleared up in time for camp. We ask that you begin doing the same at home. If head-lice has been an issue at home, please contact your local Salvation Army representative to see if help for this can be made available to you. Please note that even if medical personnel (such as your child s physician or nurse) states that your child is clear of nits or head-lice, your child will still need to be screened by Salvation Army representatives before departure and upon arrival at camp. If evidence of nits/head-lice is found on your child by Camp Walter Johnson screeners, your child will have to return home with the driver/chaperone, regardless of any prior statements by medical or Salvation Army personnel. We will not conduct treatment for head-lice at camp no exceptions.

3 The Salvation Army Camp Walter Johnson Parent Information p. 3 of 3 GUIDELINES FOR CLOTHING AT CAMP Your child will need enough clothing for each day of camp, plus an extra change of clothing if possible. It is helpful to LABEL your child s clothing with a Sharpie marker. A specific list of clothing needs is including with the camp packing list in the brochure. Please note the following requirements for clothing: No clothing with tobacco, alcohol advertisement or offensive designs or words may be worn at camp. All shorts/pants must cover underpants and stomach area (with shirt) at all times. No sagging. No holes in shorts/pants that reveal underclothing Shirts/dresses for girls: No spaghetti-strap tops/camisoles for girls. If tank style top, strap must be as wide or wider than the width of two fingers. Must not reveal bra straps, or any part of bra. Tops must also not reveal any cleavage or breast area either when standing or bending forward. Tops must be long enough to cover tummy when arms are raised high. Swim suits: All campers will need a swim suit. Campers cannot swim in regular clothing or shorts. Swim suits for GIRLS must be one-piece or completely cover tummy when arms are raised. They must NOT be low-cut or have cut-out holes. Any swimsuits that do not have adequate coverage will need to be covered by a dark-colored t-shirt in and out of the swimming pool. Shoes: Campers may bring flip-flops to wear to and from the pool, but they must also have shoes that close around the foot that can be worn in sports, hikes, etc. PLEASE DO NOT SEND CHILDREN TO CAMP WITH ONLY FILP-FLOPS. Salvation Army uniforms & Native American/Indian costumes: Children who are involved in music conservatory, Junior Soldier camp, Adventure Corps Explorers/Rangers (SAAC), Girl Guards or Sunbeams are ENCOURAGED to bring their uniforms to wear to camp worship services, but PLEASE LABEL EACH PART of these uniforms with your child s name & corps. Girl Guards & Sunbeams should also bring their Native American dresses and accessories that they have made during previous camp sessions. Please also label these items accordingly. If you are uncertain as to whether any of your child s clothing meets with Camp Walter Johnson standards, please check with your local Salvation Army representative for clarification and assistance. WHAT NOT TO BRING TO CAMP In order for your child to truly benefit from the experience of camp, these items must not be brought to camp with your child. Cell phones ipods or other electronic music devices Digital Cameras (disposable cameras are fine) Medicines (prescription or over-the-counter) that are not listed on the Health History Form Money Knives, weapons, butane lighters, fireworks nor any other incendiary (flammable) device Your child will be solely responsible for anything he/she brings to camp in the way of clothing/jewelry or personal items. Please consider keeping expensive clothing or jewelry at home while your child attends camp.

4 Camp Forms Check-Off List This is for you to keep track of the forms to be filled out. You do not need to turn in this page. This will help your child s belongings make it back home. Pack pillows, bed linens & towels in a heavy duty garbage bag, tied shut. Please pack all medicines for each child in ONE zip lock bag, labeled with your child s name/city/county on it. Sleeping bag or sheets and blanket for twin bed Pillow Towels & washcloths (2 of each) Soap & shampoo Toothpaste & toothbrush Hairbrush/comb Shower shoes (flip flops) Bathing suit (no 2 piece suits or cut off shorts) Sunscreen Bug Spray Pajamas Change of clothes for the number of days at camp, plus extra if possible Messy games clothes old Tshirt, shorts that can get messy or muddy Underclothes for number of days at camp, plus extra Closed-toed shoes (for running, hikes, etc.) Socks for number of days at camp Pre-addressed & stamped postcards for writing home Medicine (packed separately in a zip lock bag turn in to corps officer or chaperone)

Will serve lunch to campers who are staying. 1-3 pm, if only bringing campers and not picking up Boys & Girls Club & Community 2 July :30-3 pm, no lunch served 9:00 10:00 am Youth Camp July :30-3 pm, no lunch served 9:00 10:00 am Sunbeam Camp July :30-3 pm, no lunch served 9:00 10:00 am BEFORE LEAVING FOR CAMP FAX any final changes in your registration to camp. We will make every effort to adjust your registration and cabin assignments prior to your arrival in order to speed the time it takes to check your campers. Have all forms grouped and alphabetized as indicated below. For the first week of camp, please group Conservatory campers forms separately from Jr Soldier campers forms. Conduct final head-lice exams before campers and their belongings are loaded into the van. Collect campers medications, placed in zip lock bags labeled with campers name & corps/club. Medicine should NOT be packed in campers luggage. Corps officer/leaders must collect this. Make sure campers have not brought cell phones, digital cameras, or other electronics/valuables. Label luggage/tote bags with campers name & corps/club. Place pillows/sleeping bags in sturdy trash bags labeled with child s name & corps/club (use masking tape for dark bags) Pray with campers & parents for departure. Campers give goodbye hugs to parents and BUCKLE UP in the van for a safe & fun ride to camp!!!! ARRIVAL PARKING: Cars, minivans, 12-passenger vans - park in gravel lot across from Shelter A. Buses park beside Welcome Center STOP 1 Shelter A Leave luggage in the vehicle until all campers are checked completely through registration and are ready to go to cabins. Please sign in with Summer Staff, who will be on hand to help entertain your campers while your camp forms are given a preliminary check for completion and order. STOP 2 Holz Hall Camper Health Screening Bring campers to the right side door/tent and give head lice forms to health screening staff. Campers will be given a health screening and checked for head lice here. STOP 3 Holz Hall Camper Check In - While campers are at the Health Screening station, bring all other forms and corps check into the main room. Campers will come into the main room and be supervised by summer staff after they complete their health screening. After forms, registration fees, and health screenings are complete, their belongings can be unloaded to camp trailers to be delivered with campers to their cabins. Please have forms grouped according to the stations below, then in alphabetical order. Please have Music/Arts Conservatory forms separate from Jr Soldier camper forms. Station 1: Camper check in & cabin assignments Have health forms available for reference to verify that we have correct names, birthdates & gender of all campers being checked in. Station 2: Corps Check for Camper Registration and Corps/Club Contact Form Station 3: Permission waiver forms & Challenge Course forms Station 4: Summer Food Service Free & Reduced Meal Application Forms Station 5: Health forms & immunization records Station 6: Camper medications

18 Corps/Club Camp Session Camper/Program Staff Allergies & Dietary Restrictions Camp Walter Johnson 2015 Instructions: Please make copies as necessary so that you have a blank form for EACH SESSION of camp that you will be sending campers. Provide the camp session and list only the names of campers or program staff members who have allergies or dietary restrictions. You only need to list the campers who have allergies. FAX this form to camp at least FOUR DAYS prior to the start of each camp session. 19 Camper Head Lice & Nit Exam Report Camp Walter Johnson Instructions: Make a copy of this form for every camper attending camp. Fill out sections of this form at various times as indicated, including the final pre-screening on the day that the camper leaves to go to camp, before he/she gets on the bus to go to camp. This form should be completed by the person who is conducting the exam. If possible, conduct departure-day head checks before loading campers belongings and linens on the bus to help prevent possible transmission of lice to the belongings of other campers. Name of product date If found to have head lice/nits, this child will require a second exam one week later, as well as a 2 nd treatment one week following the first treatment. Please indicate date of 2 nd treatment and complete the report on the 2 nd exam below Week BEFORE the child attends camp: (This exam is not necessary if child was lice/nit free prior week) This camper has been examined for head lice and nits on (Date of exam) by (Name of person conducting the exam), and has been found to have: No head lice and no nits Head lice or one or more nits & his/her head was treated with on. Name of product date If camper is found to have head lice/nits one week prior to camp, please work with parent to consult physician as to how best to continue treatment for the child. Please contact Major Julie Whiten to discuss the progress you are having in getting the situation under control. We want to offer guidance as we are able, in order to increase the probability for the child to attend camp Morning of departure before the child attends camp: (This exam should be conducted BEFORE the child s luggage and linens are loaded onto the vehicle, if possible.)
