OCCUPATIONAL SAFETY & HEALTH COMPLIANCE The management of successful companies shall give high commitment to ensure the safety and health of their workers, transporters, clients or users in accordance with Occupational Safety & Health Act (OSHA 1994) and Factories and Machinery Act (FMA 1967). These include but not limited to the following items: 1.1 Comply with Section 15: Occupational Safety And Health Act 1994 i. The management of successful companies shall ensure, so far as is practicable, the safety, health and welfare at work of all his workmen (employees). ii. Without prejudice to the generality of item (i), the matters to which the duty extends, so far as is practicable, include in particular: a. The making of arrangement for ensuring safety and absence of risks to health in connection with the use or operation, handling, storage and transport of substances. b. The provision for such information, instruction, training and supervision as is necessary to ensure the safety and health at work of his workmen c. The provision and maintenance of a working environment for his workmen that is safe without risk to health, and adequate as regards facilities for their welfare at work. 1.2 Personal Protective Equipment Provision and maintenance of Personal Protective Equipment to all his workmen with adequate quantity and of approve quality. The equipment shall include but not be limited to the following: i. Safety boots ii. Safety helmets iii. iv. Safety harness Protective glove v. Safety Goggles vi. Ear plug and muffs vii. Dusk masks viii. Respirator

7 1.3 Permit to Work Approval The management of successful companies will have to get a Permit to Work (PTW) from PTJ involved before allowed to carry out the supply. The management of successful companies has to fill up the PTW s form and must complies the Occupational Safety and Health regulation before work permitted by the PTJ s Officer or representative. UMP is entitled to impose any action pursuant to the incompliance of Occupational Safety and Health Act 1994 and Factories and Machinery Act (FMA 1967). 1.4 Safety Data Sheet (SDS) The management of successful companies shall furnish a SDS for each hazardous chemical supplied and any chemical mixture containing hazardous substance at the concentration exceeding the cut-off value of concentration for the relevant hazard class.
