This information is published to inform the aviation industry and the public of the general circumstances of accidents and serious incidents. The information is subject to alteration or correction if additional evidence becomes available. All times given in this bulletin are Coordinated Universal Time (UTC). East African Local Time is UTC plus 3 hours. P.O. Box Nairobi Telephone: Fax:
MD-11F PH-MCR Martinair Cargo MD-11F Mcdonnell Douglas Corp/ Boeing Year of manufacture 1994 PH - MCR Aircraft serial number Date of registration 30 th March 1995 Pratt & Whitney PW4460 Date of incident 13 th January hours RWY 06 Jomo Kenyatta International Airport Public Transport - Cargo Number of persons on board Crew 3 Passengers Nil Injuries Crew None; Passengers None Aircraft substantially damaged Captain s flying experience hours (of which 7895 were on type) Damaged flap The Dutch aircraft, registration number PH MCR, a Martinair Cargo flight from Amsterdam to Nairobi had a runway excursion on landing at Jomo Kenyatta International Airport (JKIA). The aircraft skidded and the left main landing gear ran in the grass area for about 500 meters before the aircraft was brought back to the runway. It was raining at the time and the runway was wet. The remainder of the landing roll was uneventful and the aircraft was taxied to the parking area. Approach was initiated within the requirements of the Operational Manual, i.e. within visual and weather limits. The weather information available to the crew did not necessitate the captain to execute the approach and landing. The probable cause of this incident was the F/O s failure to recognize the magnitude of the increasing drift angle, leading to insufficient correction. The captain took over controls and despite his efforts to arrest the lateral movement of the aircraft, the left hand wheels, and gradually all wheels of the left hand bogie left the paved surface. Eventually the captain managed to recover control and steered the aircraft onto the runway again. 2
Piper PA Y-CUB Owner/operator Alexander Prousek Piper PA Super Cub Piper/Cub Crafters Inc. Year of manufacture Y - CUB Aircraft serial number 9952C Date of registration 5 th October 2006 Lycoming O-360-C4D Date of accident 26 th January 2007 Time of accident 1430 hours Location of accident Chololo Farm, Laikipia District Private Number of persons on board Crew 1 Passenger 1 Injuries Crew Fatal; Passenger Fatal Aircraft destroyed Commander s flying experience 102 hours (of which about 25 were on type) The aircraft departed Ol Jogi Airstrip at about 1335 hours for a local flight around the area. On board were the pilot and one passenger, both foreign nationals. The next time it was seen was between 1400 and 1430 hours by the owner of Chololo Farm. It flew overhead for a while then disappeared behind the hill. When the aircraft did not return to the Airstrip that evening, the manager of Ol Jogi Farm called ATC at Jomo Kenyatta International Airport and reported the matter. The following morning a search was mounted using a helicopter and the wreckage was found about 11 kilometers from the Airstrip. Both occupants were still strapped in their seats but fatally injured. There was no fire. The probable cause of this accident was loss of engine power during flight maneuvering, leading the aircraft to dive and impact the ground. 3
Cessna 208B 5Y-BLN East African Air Charters Limited Cessna 208B Cessna Aircraft Corporation Year of manufacture Y - BLN Aircraft serial number 208B0558 Date of registration 21 st February 2001 PT6A-114A Date of incident 16 th May hours Wilson Airport Apron Public Transport Number of persons on board Crew 1 Passengers Nil Injuries Crew None; Passengers None Propeller blades bent and left wing tip damaged Captain s flying experience 7500 hours (of which 1000 were on type) The aircraft was on a positioning flight from Wilson Airport to Jomo Kenyatta International Airport with only the captain on board. On taxiing out, the aircraft hit the customs building. The pilot said that she lost directional control and suspected that the brakes were faulty. The aircraft was towed to the hangar and the brake system checked. No anomalies were noted. The probable cause of this incident was distraction of the pilot during taxiing out. 4
Cessna 150F 5Y-BSK Cessna 150F Cessna Aircraft Corporation Year of manufacture Y - BSK Aircraft serial number Date of registration 10 th January 2005 Continental O-200-A Date of incident 4 th June hours Kijipwa Airfield Flight Training Number of persons on board Crew 1 Passengers Nil Injuries Crew None; Passengers None Nose wheel oleo broke off and propeller blades bent Pilot s flying experience 31 hours (of which 6 were on type) The student pilot was flying solo doing circuits at the Kijipwa Airfield. The aircraft rolled for about 15 meters then the nose wheel collapsed and the aircraft came to an abrupt halt. 5
Cessna F172L 5Y-APH Cessna F172L Cessna Aircraft Corporation Year of manufacture Y - APH Aircraft serial number F Date of registration 27 th July 2001 Avco Lycoming O-320-E2D Date of incident 22 nd June hours Lembus Forest, Koibatek District Private (Navex) Number of persons on board Crew 2Passengers Nil Injuries Crew Minor; Passengers None Aircraft destroyed by impact with trees Captain s flying experience hours (of which 500 were on type) The aircraft was on a navex flight from Wilson Airport with the captain and copilot on board. The aircraft, type Cessna F172L, was operated by Aeronav Limited. While flying over Lembus Forest in Koibatek District, the aircraft hit trees on the side of a hill. The hill was covered in fog. IMC prevailed during the flight. The pilot was hospitalised for several days. The copilot received minor injuries. The aircraft was completely destroyed by the impact with the trees. The probable cause of this accident was controlled flight into terrain (CFIT). The weather/poor visibility was a major contributory factor. 6
Cessna A152 5Y-GSJ Ninety Nines Flying Club Cessna A152 Cessna Aircraft Corporation Year of manufacture Y - GSJ Aircraft serial number A Date of registration 2 nd August 1999 Avco Lycoming O-235-N2C Date of incident 24 th June hours Runway 07 Wilson Airport Flight Training Number of persons on board Crew 1 Passengers Nil Injuries Crew Minor; Propeller spinner and vertical stabilizer damaged. Windscreen broken. Pilot s flying experience 37.5 hours (of which 37 hours were on type) The student pilot was authorized to carry out a solo circuit flight by his instructor after a 30 minutes pre-solo check out. While on his first circuit flight, the student requested ATC for a touch-and-go and was given permission. He touched down slightly to the left of the centerline and the aircraft immediately veered off the runway into the grass. As he tried to stop the aircraft, the right wheel hit a metal obstacle and the aircraft overturned. The student received a small cut on the finger and knocked his forehead. 7
Cessna FR172G 5Y-AMP Z. Boskovic Air Charters Limited Cessna FR172G Cessna Aircraft Corporation Year of manufacture 1 st January Y - AMP Aircraft serial number FR Date of registration 30 th May 1985 TCM IO-360-D20-B Date of incident 3 rd July hours Loita Hills, Narok DistrictS E Public Transport Number of persons on board Crew 1Passengers 2 Injuries Crew Fatal; Passengers Fatal Aircraft destroyed by impact with rocky hill Captain s flying experience hours (of which were on type) The aircraft departed Jomo Kenyatta International Airport at 0453 hours with an ETA at Siana Springs, Maasai Mara of 0553 hours. On board were the pilot and two passengers. The plane did not arrive at Siana Springs. Several hours later the wreckage was found on the nearby Loita Hills at an altitude of approximately 7600 feet. All on board had perished. The probable cause of this accident was controlled flight into terrain (CFIT) due to the pilot s lack of vertical situational awareness. A major contributing factor was the poor visibility that prevailed in the Loita Hills area at the time of the accident. 8
Beechcraft BE 1900C 5Y-BTT Aero Kenya Limited Beechcraft BE 1900C Raytheon Aircraft Corporation Year of manufacture 8 th January Y - BTT Aircraft serial number UC Date of registration 13 th February 2007 PT6A-65B Date of incident 6 th July hours RWY 07 Wilson Airport Public Transport Number of persons on Crew 2Passengers 19 board Injuries Crew None; Passengers None Aircraft substantially damaged Captain s flying experience hours (of which 50.0 were on type) The aircraft had a runway excursion on landing at Wilson Airport. It veered off to the left and came to a stop in the grass area 100 meters from the end of Runway 07. The two crew and 19 passengers were not injured. It was determined that the approach to landing was fast and high, leading to the aircraft touching down too far down the runway. 9
