Officer gained entry to the vehicle and the waiver was completed. Driving Complaint 06/19/ :00: SKOKIE HWY Officer responded to the area of southbound Route 41 at Route 176 for a report of three vehicles travelling at a high rate of speed, passed on by North Chicago Police Department. Officer checked from Route 176 to Deerpath Road. The vehicles were gone prior to officer s arrival. Animal Complaint 06/19/ :05: E SHERIDAN PL / EVANSTON AVE Officer responded to the area of East Sheridan Place and Evanston Avenue for a report of a dog at large. Officer searched the area but was unable to locate. Animal Found 06/19/ :47: E CENTER AVE Officer responded to the PSB for a walk-in report of a found animal. The dog s tags had a phone number on it. The owner was contacted and responded to the PSB to pick up the dog from Briar Lane. A verbal warning was issued to the owner for dog at large. Lost/Mislaid Property 06/19/ :48: E CENTER AVE Officer took a walk in report of a stolen cell phone. Complainant stated that the phone was taken out of her purse at either Dunkin Doughnut or Jewel in Lake Forest. Officer called dispatch and requested Lake County Sheriff s department to be notified. A Deputy arrived on scene and took the report in the lobby of the Public Safety Building. Public Health and Safety 06/19/ :20: GREEN BAY RD / ROCKLAND RD Officer responded to the intersection of Green Bay Road and Route 176 for a refused call of the crossing guard missing children crossing. Officer spoke with the crossing guard, who appeared to have a clear vantage point of the intersection. The guard noted not recalling missing any crossings. 911 Call Investigation 06/19/ :38: ROCKLAND RD Officer responded to a business on Rockland Road for a report of a 911 hang-up. Officer spoke with an employee on location who stated a juvenile called 911 from a guest phone. Officer initially spoke with the juvenile, who denied the incident. Officer checked the phone, and it would call out to 911. Officer notified the business management. Officer made contact with the juvenile and guardian who would not fully admit to the incident. Officer confirmed there were no concerns and cleared. Driving Complaint 06/19/ :23: SKOKIE HWY Officers responded to area of Rt. 41 at Rt. 176 for a report of a black Land Rover on N/B Rt. 41 driving recklessly. Officers were unable to locate the vehicle. No complaints to be signed.

2 Motorist Assist 06/19/ :37: S WAUKEGAN RD / CARRIAGE PARK LN Officer located a disabled vehicle on Rt 43 north bound at Carriage Park Lane. The driver stated that he already had a tow truck on the way and did not need any assistance. Officer assisted the driver moving the vehicle onto Carriage Park Ln. so it was not a hazard. Assist Fire 06/19/ :47: LINCOLN AVE Officer responded to the 400 block of Lincoln Avenue for a report of smell of gas by the homeowner. Upon arrival, officer observed moderate odor of natural gas near the fireplace. LBFD and LFFD arrived on scene and were able to air out the chimney and check the couplers. No further assistance required, however, homeowner was advised to have a plumber check the pipes, valves and seals. Animal Complaint 06/19/ :09: SIGNE CT Officer responded to the 300 block of Signe Court for a large turtle in the back yard. Officer was able to remove the turtle and put it outside their fence in a wildlife area. Trespassing 06/19/ :36: ARDEN SHORE RD Officer located four juveniles on the vacant private property lot on Arden Shores Rd. Three of the subjects met with the officer and were issued no trespassing warnings and were released to parents. One juvenile obstructed a peace officer by running from the scene and failing to comply with lawful order. The subject was later caught and issued citations for unlawful consumption of alcohol by a minor and obstructing a peace officer. Suspicious Incident 06/20/ :12: E WASHINGTON AVE Officer responded to the 500 block of East Washington Avenue for a report of a vehicle in the driveway with the lights on reported by the paper delivery driver. The driver stated he did not see anybody else in the area, he just thought it was odd the lights were on. Officer checked the exterior of the vehicle and illuminated the interior of the vehicle, no irregularities were observed. Officer attempted to make contact with the homeowner but was met with negative results. Officer left a crime prevention form on the windshield of the vehicle. Animal Complaint 06/20/ :12: SKOKIE HWY Officer responded to northbound Route 41 near a dealership for a report of several geese along the roadway. Complainant was concerned that the birds would cause an accident. Officer checked the area and was unable to locate. Traffic Stop 06/20/ :33: GREEN BAY RD / ROCKLAND RD Officer conducted a traffic stop on Green Bay Rd south of Route 176. The driver was determined to have a revoked driver s license and was arrested for failure to wear a seatbelt, operating an uninsured vehicle, and driving while license revoked. The subject was turned over to Lake County Sheriff's Department to be held for bond court. Ambulance Assist 06/20/ :18: PHILLIP CT Officer responded to the 100 block of Phillip Court for a report of a subject not feeling well. Lake Bluff and Lake Forest Fire Departments responded. The patient was transported to Lake Forest Hospital. The healthcare worker on location notified the subject's spouse and family. Assist Fire 06/20/ :08: NEUMAN CT Officer responded to the 300 block of Neuman Ct. for a fire alarm. Upon arrival, Officer observed no irregularities and no one was home. KFD and LBFD arrived on scene and a keyholder was located and responded. Unknown cause for the alarm. A keyholder contacted the homeowner and they will speak with the alarm company

3 Motorist Assist 06/20/ :09: S WAUKEGAN RD / SHERWOOD DR Officer responded to Rt. 43 at Sherwood Drive for a vehicle that ran out of gas. Driver had her father respond to the scene and was able to get it moving again. Citizen Assist 06/20/ :17: W CENTER AVE Officer met with a subject in the PSB in regards to people standing in front of her at last year s 4th of July Parade. The subject was very frustrated due to arriving for the parade early and then not being able to see it. I advised the subject that I would pass along her concerns to hopefully avoid any further problems at this year s parade. Citizen Assist 06/20/ :16: W WASHINGTON AVE Officer responded to the Park District after a juvenile camp attendee had a neighbor attempt to pick him up. The neighbor was not on the accepted list and the Park District couldn't contact the parents. The child was not released to the neighbor and the police were called. However, prior to Officer arrival, parents were contacted and were sending an uncle to pick up the child. No further assistance needed at this time, child was with staff until uncle arrived. Lock Out 06/20/ :23: ROCKLAND RD Officer responded to a business parking lot for a vehicle lock out on a semi-truck. Officer arrived on scene and determined that that entry would not be possible by this department. The driver stated he would make other arrangements and that no further assistance was needed. Public Health and Safety 06/20/ :36: SKOKIE HWY Officers responded to area of n/b Rt. 41 at a dealership for report of a goose hit by a car and multiple geese in the roadway causing a hazard. Officers located the geese at Gage and notified LFPD. Officers assisted LFPD with shagging the geese off the roadway, however they kept coming back in other locations near Gage. Officers were unable to do anymore for the geese. Animal Complaint 06/20/ :02: E CENTER AVE Officer responded to the 200 block of E. Center Ave for a report of a found dog that was running loose. Officer arrived on scene and took possession of the dog. Prior to officer leaving, the owner was located and the dog was returned. Officer issued C for dog at large. Alarm Business 06/20/ :52: SHERWOOD TER Officers responded to the 30 block of Sherwood Terrace for a report of an activated burglar alarm. Officer arrived on scene and met with an employee who stated he was unsure of why the alarm was going off. Officers checked the interior noting no irregularities. The employee reset the alarm and officers cleared. Trespassing 06/20/ :50: MAPLE AVE Officer responded to Lillian Dells after a resident complained of 6-8 juveniles on the property 15 minutes after Sunset. Officer located six juveniles, all on their way back up the path towards the exit. The juveniles were from Libertyville, Mundelein and Lake Forest. All were issued verbal warnings for trespass and shown the sign for accepted hours. All had no previous contacts at this location. Three of their vehicles were issued parking citations for parking east of Moffett Road after 9PM.

4 Citizen Assist 06/20/ :37: SIGNE CT Officer responded to the 300 block of Signe Court to assist the homeowner with a broken garage door. The homeowner stated she is not able to lower the door and is concerned leaving it open for the night. Officer was able to lower the door for the homeowner. Ambulance Assist 06/21/ :18: NORTH SHORE DR Officer responded to a business in the 900 block of North Shore Drive for a report of a subject feeling ill. Lake Forest Ambulance, Lake Bluff Fire Department, and Knollwood Squad responded. The patient was transported to Lake Forest ER. Public Health and Safety 06/21/ :55: W SCRANTON AVE / MAWMAN AVE While on patrol, officer was flagged down by a passerby who stated there was a large box near the roadway on West Scranton Avenue east of Mawman Avenue and she was concerned that it may have been lost property. Officer located the box and determined it was empty. The hazard was removed. Vehicle Theft 06/21/ :12: NORWICH CT Officers responded to the 200 block of Norwich Court for a report of a vehicle stolen sometime since 06/20/16. Alarm Business 06/21/ :07: SHERWOOD TER Officer responded to the 30 block of Sherwood Terrace for an activated panic alarm. Officer arrived on scene and spoke with the manager of the building who stated that the alarm was set off in error. Burglary to Vehicle 06/21/ :52: NORWICH CT Officer responded to 100 block of Norwich Ct for a burglary to motor vehicle. Officer arrived on scene and spoke with the complainant who stated that her sunglasses and case were missing from the center counsel of the vehicle. The glasses were described as brown rimmed Ray bans and the case was homemade needle point multicolored. Officer advised the complainant that the glasses might be the same as the pair found in the yard in the 200 block of Norwich Ct., incident number Accident PP 06/21/ :40: S WAUKEGAN RD Officer responded to the 200 block of S Waukegan Road for a 2 vehicle private property accident. Officer completed an SR-1050 traffic crash form and gave a copy to each driver along with an explanation. Ambulance Assist 06/21/ :02: S WAUKEGAN RD Officers responded to the 100 block of S. Waukegan Road for an unresponsive subject. Lake Bluff, Lake Forest, and Knollwood Fire Departments responded. The subject became alert and was transported to Lake Forest Hospital. Identity Theft 06/21/ :34: W SHERIDAN PL Officer took a walk-in report of an identity theft. Theft Under $300 06/21/ :56: SUNRISE AVE Officer took a station report of a stolen bicycle from Sunrise Beach.

5 Alarm Residential 06/21/ :05: PARK LN Officers responded to the 300 block of Park Lane for an activated burglar alarm. Officer met with the homeowner who had set the alarm on accident. Officer observed no irregularities. Animal Complaint 06/21/ :54: SHERWOOD DR Officer responded to the 900 block of Sherwood Terrace for a report of a dog locked in a vehicle with the windows rolled up. Officer met with the complainant who stated that she observed the dog barking repeatedly. At one point she stated she observed the dog laying on the floor of the car, not moving. She stated she became worried that the dog wasn't breathing. Officer met with the vehicle owner's boyfriend who stated they had rushed their 1 year old daughter to the doctor because of a medical emergency. He stated they had been at the doctor for approximately 1/2 hour and had cracked the windows for the dog. When Officer arrived, the dog had a cup of water as well. I advised the couple not to leave a dog in a vehicle during extreme temperatures for any extended amount of time. Officer verified with the complainant that she did not want to sign complaints and cleared. Alarm Business 06/21/ :25: NORTH SHORE DR Officers responded to the 900 block of North Shore Drive for a burglar alarm. Officer found all doors secure and met with the keyholder and vice president of the company, who stated he allowed access to the building for contractors who were doing work on the roof and brick above the rear door. The contractor advised that he will be working at that location all week. Suspicious Incident 06/21/ :15: SHERIDAN RD Officers responded to the Train Depot for a report of multiple subjects in a parked car drinking beer. Upon arrival, Officers did not locate any occupied vehicles. A train had left the station as Officers arrived. Alarm Residential 06/21/ :14: WARRINGTON DR Officers responded to the 80 block of Warrington Drive for a burglar alarm; overhead garage door activation. Upon arrival, Officer met with the homeowner who had set the alarm off in error. Motorist Assist 06/21/ :30: ROCKLAND RD Officer responded to Rt 176 east of Public works for a vehicle that had overheated and broke down. Officer gave a courtesy ride home to the elderly driver, while waiting for the tow company to arrive. The towing company arrived at 2105 hours, removed the vehicle, and transported it to the driver's residence. Premise Check 06/21/ :09: ARDEN SHORE Officer conducted a premise check of the Arden Shore property and located two juveniles from Libertyville on the beach. Subjects cleared LEADS/NCIC and were issued written warnings for No Trespass. The parents of both subjects were notified. Motorist Assist 06/21/ :53: ROCKLAND RD / GREEN BAY RD Officer responded to the area of Route 176 at JAWA for a report of a disabled vehicle backed into a ditch along the north side of the roadway. Officer checked the area but was unable to locate the vehicle.

6 Parking Problem 06/22/ :30: WARRINGTON CT Officer responded to Warrington Court for a parking complaint, vehicles blocking the roadway. Officer observed no violations. Lock Out 06/22/ :57: ALBRECHT DR Officer responded to the 100 block of Albrecht Drive for an auto lock out. Officer arrived on scene and met with the complainant who was run through LEADS/NCIC and was the registered owner of the vehicle. Waiver form was signed and entry gained. Motorist Assist 06/22/ :52: SHERIDAN RD / CRAB TREE LN Officer observed a vehicle parked on the side of Sheridan Rd at Crabtree Farms with the driver standing outside of the vehicle. Officer met with the driver who stated he pulled over after receiving some bad news of a friend that died. He didn't want to drive while he was crying. The driver stated he didn't need any further assistance and then left the area. Suspicious Incident 06/23/ :37: PINE CT Officers responded to the 400 block of Pine Court for a complaint from a person out of town that the resident heard the doorbell ring and scratching at the front door. Officers checked the residence exterior and the surrounding area and did not locate any irregularities. Assist Other Agency 06/23/ :19: S WAUKEGAN RD / ROCKLAND RD Officer located a vehicle possibly involved in a driving complaint that originated in Lake Forest Police jurisdiction. Officer followed the vehicle east on Rt. 176 from Rt. 43 and then north on Green Bay Road to Signe Court and observed no traffic violations. Ambulance Assist 06/23/ :22: S WAUKEGAN RD Officer responded with the Lake Bluff, Knollwood, and Lake Forest Fire Departments to the 70 block of S. Waukegan Road for a patient not feeling well. Patient was transported to the Lake Forest Hospital in ambulance # Call Investigation 06/23/ :01: W SHERIDAN PL Officer responded to the 200 block of W. Sheridan Place and verified that a 911 call was accidental by the resident s child. Lock Out 06/23/ :45: OAK AVE / E NORTH AVE Officer responded to Oak at North for an auto lockout. Waiver signed, entry gained. Animal - Carcass 06/23/ :17: ARBOR DR Officer responded to the 600 block of Arbor Drive for a resident that located a deceased bat. LC Animal control was notified by voic . Resident was able to dispose of bat and Officer cleared.

7 Child Seat 06/23/ :45: E CENTER AVE Subject came into the Lake Bluff PSB for a child safety seat install. An Operation Kids Child Safety Seat Checklist was filled out and seat was installed. Driving Complaint 06/23/ :05: SKOKIE HWY Officer responded to the area of s/b Rt 41 at Rt 176 for the report of a black Chevy station wagon travelling at a high rate of speed from Buckley. While en route, Dispatch advised that the vehicle was observed at Rt 41 and Westleigh Rd in Lake Forest. Suspicious Person 06/24/ :28: SIGNE CT Officers responded to the 300 block of Signe Court for a report of a suspicious person knocking at the residence and requesting rope for his vehicle stuck near the property. Upon arrival, officer determined that the subject had driven off the roadway into a ditch and became lodged on some boulders. A formal traffic crash report was completed. The subject was subsequently arrested for improper lane usage and driving while license suspended. The vehicle was towed and the subject was released on a $1, personal recognizance bond with a mandatory court date. Found Property 06/24/ :15: E CENTER AVE Officer responded to the Lake Bluff Police Department for a subject in the lobby who found a purse under the West Scranton Avenue bridge, in the roadway. Officer recovered the purse from the complainant. Officer found inside the purse the owners driver's license, library card, high school ID, a small amount of currency and other miscellaneous items. The owner was contacted and stated the last known location of the purse was inside her friend s car who lives in Lake Forest. The owner was advised the purse will be placed in found property and to respond anytime to pick up the purse. Ambulance Assist 06/24/ :38: SHERIDAN RD Officer responded to the Lake Bluff Bike Path across from Crab Tree Farm for a report of a cyclist who fell off her bike. Lake Forest Ambulance A421 responded and transported the subject to Lake Forest Hospital for further evaluation. Animal Found 06/24/ :26: SUNRISE AVE Complainant walked into the Public Safety Building with a dog that had followed her from Sunrise Beach. Officer took possession of a male short haired brown and white colored dog and transported the animal to the Knollwood Animal Hospital. Found Property 06/24/ :55: E SCRANTON AVE / E CENTER AVE A complainant responded to the Public Safety Building and turned in a found wallet that he had found at the Farmer's Market. The owner of the wallet was contacted and responded to the PSB to claim the item. Citizen Assist 06/24/ :17: SKOKIE HWY Officer responded a business on Skokie Highway for a VIN verification. Verification was provided. Lost/Mislaid Property 06/24/ :36: GREEN BAY RD / BATH AND TENNIS CLUB RD A complainant walked into the Public Safety Building to report her wallet was placed on the roof of a car at a party last night and fell off somewhere between Lake Bluff and Lake Forest. She stated her friend's wallet was also placed on the roof of the same car and that wallet was located on W. Scranton Avenue. (See incident # ). Officer gave the complainant an incident number to assist with obtaining a new driver's license. No other high value items were inside the lost wallet.

8 Alarm Residential 06/24/ :22: RAVINE FOREST DR Officer responded to the 200 block of Ravine Forest Drive and verified that an activated burglar alarm was accidental by the home owner's daughter. Officer observed no other irregularities. Driving Complaint 06/24/ :34: ROCKLAND RD / GREEN BAY RD Officer responded to Green Bay Rd north of Rt 176 for a driving complaint of a silver pick up driving erratically. Officer checked Greenbay Rd to our north limits and was unable to locate the vehicle. Accident Property Damage 06/24/ :16: SHERIDAN RD / N MCKINLEY RD Officer responded to Sheridan Rd and E. Sheridan Rd for a property damage vehicle accident. A formal SR report is on file. Accident Property Damage 06/24/ :19: SKOKIE HWY / W WASHINGTON AVE Complainant responded to the Public Safety Building with another driver requesting assistance with a minor traffic crash that occurred on W. Washington Avenue at the N/B Rt. 41 entrance. Complainant's Cadillac Escalade was rear ended by the other party and they requested an incident number for insurance purposes. Officer verified all parties had valid driver's license and insurance. No other police service requested. Theft Under $300 06/24/ :02: ROCKLAND RD Officers responded to a business in the 900 block of Rockland Avenue for a retail theft in progress. One subject was arrested. Accident Property Damage 06/24/ :20: GREEN BAY RD / ROCKLAND RD Officer responded to Green Bay Road south of Route 176 for a report of a disabled vehicle in the roadway. Officer arrived on scene and determined the vehicle had been involved in a minor accident. Officer met with both parties involved and both stated that they did not require a report at this time. The driver of the disabled vehicle was able to move the vehicle onto a side street where it was not a hazard. Animal Found 06/24/ :46: E CENTER AVE Officer responded to the lobby of the PSB for a found dog. Officer was advised where the dog lived and the dog was returned home. Driving Complaint 06/24/ :41: W WASHINGTON AVE Officer responded to the 300 block of W. Washington Ave for a report of a vehicle that was speeding through the parking lot and honking the horn. Upon arrival, Officer was unable to locate the vehicle. Alarm Business 06/24/ :48: W SCRANTON AVE Officer responded to the 10 block of W. Scranton Ave for a burglar alarm. Proper codes were given before Officers arrived on scene. Ordinance Violation 06/24/ :30: SUNRISE AVE Officer responded to the 400 block of Sunrise Ave for landscapers cutting grass past hours. Upon arrival, Officer did not locate any landscapers in the area and cleared.

9 Suspicious Veh 06/24/ :39: ARDEN SHORE DR While on patrol, officer locate a vehicle with several juvenile occupants on the vacant lot on Arden Shore Dr. Officers transported all juveniles back to the Lake Bluff Public Safety Building to be released to parents. Officer issued all subjects no trespassing warnings for the property and one juvenile non-traffic citations for unlawful possession of cannabis and unlawful possession of drug paraphernalia. Suspicious Person 06/25/ :22: NORTH SHORE DR Officers responded to the 900 block of North Shore Drive for a report of subjects looking through the windows at the cars. The keyholder advised he wanted the subjects to leave the premise. Upon arrival, officers did not locate any subjects on scene. Officers checked the building finding the exterior secure, no irregularities were reported. Found Property 06/25/ :01: ROCKLAND AVE While on patrol, officer located a bicycle in the bushes along West Scranton Avenue, just east of the 600 block of Rockland Avenue. The bicycle was brought back to the Lake Bluff Police Department and entered into found property. M.C.A.T. 06/25/ :03: WEGNER RD / DARRYL RD=LAKEMOOR Officer responded to intersection at Wegner/Darryl Rd in Lakemoor for accident investigation- MCAT. Parking Problem 06/25/ :32: E SCRANTON AVE Officer was advised of a vehicle parked in a prohibited parking spot in the 100 block of E Scranton Avenue during the car show. Officer issued C Citizen Assist 06/25/ :17: S WAUKEGAN RD Officer was approached by a female at a business in the 300 block of S Waukegan Rd that her boyfriend had broken up with her and she needed a ride back to Waukegan. Several minutes later, the subject advised the Officer she had found a ride from a friend. No further assistance was needed. Dispute 06/25/ :38: SUNRISE AVE Officers responded to Sunrise Beach for a report of a vehicle that was driving on the sand and the driver was ignoring the staff. Officer met with the beach guards who did not wish to sign complaints. Officer met with the driver who stated he had to drive onto the beach to drop off numerous heavy objects. The beach guards stated that they have an ATV that can drive on the beach in the event someone needs to move heavy objects. Officer related that to the driver who stated he would utilize that option next time. Peace was restored and Officers cleared. Animal Complaint 06/25/ :47: SUNRISE AVE Officer responded to the 400 block of Sunrise Ave for a report of a bat on the ground. Officer met with the complainants and while speaking to them the bat began to take flight and flew away on its own accord. No further Police assistance was needed. Identity Theft 06/25/ :31: BIRKDALE RD Officer responded to the 300 block of Birkdale for an identity theft report. Accident Property Damage 06/25/ :14: ROCKLAND RD / S WAUKEGAN RD Officer responded to northbound Route 43 south of Route 176 for a three vehicle crash. Formal Crash Report on file.
