1 Applied community experience Welcome to a.c.e. NEW TO ACE?? All participants MUST have OR 542 in their ISP or service agreement to participate. Call your case manager! ACE REC activities are limited to only contracted members. Others may not attend, this includes family members, spouses, boy/girlfriends, PSWs or providers from other agencies. Members MUST pay for their own participation fees. DHS rules do not allow G2G to fund these costs for members. Participants can either meet at a transportation HUB or at the venue. The Activity time is the anticipated start of the activity and does not include transport time. Tipping is NOT optional when dining out! Having fun is mandatory! We are Good-2-Go... Are You? Expansion of ACE REC Transportation! A.C.E. will now be providing transportation assistance to our events! We will be utilizing community transportation HUBs. Transportation is NOT provided from member homes to the HUB. All participants are required to secure timely transportation to/from the HUB before/after the activity. All members are free to participate, even if the activity is not in your city. Please be sure to indicate in your enrollment your preferred transportation HUB. Crafts & Cooking DSA HOURS: ~3 Event Time: 3:00 pm to 5:00 pm Crafts & Cooking... what a great combination! This on-going activity is a favorite of A.C.E. members. It may a craft night, it may a cooking night, or you may be doing both! This event is also offered in Bend. The program, now A.C.E. REC, had to shutter its doors in 2013 due to funding changes. However with another rule change in 2014 and the dedication of now A.C.E. manager and Good-2-Go Oregon s longest team member Annie Banks, we have brought back the excitement and opportunity that everyone has come to love. However, occasionally, we might like to throw a little party of our own such as the Day at the Lake event! Be sure to check out the great events that follow to see if any might tickle your fancy! Fitness club is a great opportunity to come out and enjoy some low-impact exercise, and enjoy some of our beautiful summer weather. This is to help ensure that our staff maintain a professional relationship & appropriate boundaries and reduces the possibility of any improper contact between our team members and our members. 10 ELIGIBILITY Must be at least 18 years of age Must be able to independently feed oneself Must be able to take medications independently when on G2G events Must not present a credible threat of harm to self or others. Participants must also receive 1:1 supports through Good- 2-Go Oregon. This is a Member's Only Program. PARTICIPATION FEES This is the actual cost to participate in the community activity. This cost may include such things as tickets, entrance fees, food/beverage, tips, etc. The participant must cover these costs themselves, as they are not covered by state funding agencies such as Brokerages and CDDPs. Please note the following policies on Participation Fees: Good-2-Go Oregon does not collect participation fees. They are paid directly to the vendor/venue of the event. o On RARE occasions, Good-2-Go will purchase tickets in advance when necessary. In these cases, enrollment must include price of advanced ticket sales. Advanced Purchased Tickets are Non-Refundable! Good-2-Go participants must be able to carry their own money. Staff are not permitted, under any circumstances, to carry member's money. o Staff may assist in making purchases, counting change, and collecting receipts. Good-2-Go staff cannot pay for participation fees, food/beverage, or tips. ENROLLMENT All event enrollment must be done through our website: ACE recreation program does not accept call-in or drop-in enrollments. Participation fees are not collected at the time of enrollment. CANCELLATIONS You may cancel events through the website. Please call our office for cancellations within 48-hours of the event. NO-SHOWS Individuals who no-show for events will be contacted by phone to confirm intended attendance or absence. Individuals who no-show 3-times during a newsletter cycle, will be suspended from participation in all events during the next newsletter cycle. TRANSPORTATION Members are expected to make arrangements for transportation to/from a Transportation HUB or the venue itself. This may include parents, public transit, or 1:1 staff scheduled prior/after the event. When scheduling transportation, please make sure pick-up time is within 15 minutes of scheduled event end-time. Individuals will be billed DSA time until they are pickedup from the HUB/Venue. TERMINATION Good-2-Go wants to ensure the positive experience of all of its members. As such, there are occasions when participants may be asked to not participate in future activities. The following are reasons that may result in termination from the ACE Rec Program: Physical assault on another member or a member of the community during an event (see eligibility criteria, as an off-event assault may also affect eligibility). Not participating as a full-value member. Having 3 No-shows Not following participation guidelines as outlined in the Participant Handbook. Repeatedly not having necessary funds for participation fees. Any action or choice that places the member or other members at risk of personal harm. GROUP VS 1:1 HOURS All A.C.E. events are billed to group /DSA hour (OR542). 1:1 hours may not be used for A.C.E. events. Individuals wishing to participate in the ACE Rec Program must have Group Support hours in their ISP and Service Agreement (Brokerage Clients). These services are titled Day Support Activities, OR 542 by the state. You must request these hours be included in your ISP before participating in any ACE Rec Events.
