Officers checked the grounds and observed no other irregularities. Public Health and Safety 09/27/ :18: N SHERIDAN RD / E SCRANTON AVE Narrative: Officer responded to W. Scranton and Mawman for a bush that fell off a truck. Item was already on the side of the road and the owner was loading it onto a truck. Item removed and Officer cleared. Animal Found 09/27/ :56: E PROSPECT AVE Narrative: Officer responded to the 300 block of East Prospect Avenue for a report of a dog in the front yard. The caller was holding onto the animal. Upon arrival, officer met with the caller who advised the dog was returned to the owner (in the 300 block of East Center Avenue). No police assistance was needed and officer cleared. Traffic Arrest 09/27/ :44: S WAUKEGAN RD / ROCKLAND RD Narrative: Officer responded to the area of eastbound Rockland Road from Waukegan Road for a driving complaint. The refused complainant stated the vehicle, a black BMW SUV was all over the roadway. Officer located the vehicle eastbound Rockland Road as it was approaching Green Bay Road. Officer conducted a traffic stop on the vehicle on East Sheridan Place at Evanston Avenue. The driver was subsequently arrested for improper lane usage and driving under the influence of alcohol. Suspicious Vehicle 09/27/ :05: ARDEN SHORE DR Narrative: Officer responded to Arden Shores area for a report of two occupied vehicles in the vacant property making noise. Officer arrived on scene and could not locate any subjects. Dispatch advised there was no complainant to be seen. Assist Fire 09/27/ :14: N BIRCH AVE Narrative: Officer responded to the block of North Birch Avenue for an activated carbon dioxide alarm, unknown on symptoms. Officer arrived and determined Knollwood Fire Department was already on location. Officer confirmed with the Fire Department staff that no assistance was needed Criminal Damage 09/28/ :29: E SCRANTON AVE Narrative: Officer responded to the 500 block of E. Scranton Avenue to meet with the home owner regarding damage to a tree that was planted by the Village of Lake Bluff Public Works. Officer observed damage to a small tree on the parkway due to a vehicle leaving the road way and side swiping the tree. Tire tracks were observed that led from the parkway to the south side of the tree onto the sidewalk then returned to the roadway proceeding east bound. Complainant wanted the incident documented and a request made to the LBPW to tend to the tree. No other police service requested.

2 Found Property 09/28/ :19: WEATHERFORD CT Narrative: A resident came to the PSB with an electronic "Flowmaster" she found in the parkway in the 300 block of Weatherford Ct. The item was logged into found property. Deceptive Practices 09/28/ :32: BOARDMAN CT Narrative: Officer responded to Boardman Ct for a report of fraudulent use of a credit card. Citizen Assist 09/28/ :42: SKOKIE HWY Narrative: Officer responded to a car dealership for a VIN inspection. Officer spoke with the sales personnel who stated they were conducting a registration transfer to Florida. Officer completed a VIN inspection form Animal Complaint 09/28/ :51: EVANSTON AVE / E SCRANTON AVE Narrative: A resident responded to the Lake Bluff Public Safety Building with a female Golden Retriever that was running at large near Evanston Avenue and E. Scranton Avenue. The dog was a female with a pink collar and tag with the name. A phone number was found on the tag but no one answered. The dog was taken to the Knollwood Animal Hospital. Lock Out 09/28/ :17: E CENTER AVE Narrative: Officer responded to the block of East Center Avenue for a vehicle lockout. Upon arrival, officer met with the caller who stated a relative brought her another key for the vehicle. No police assistance was needed and officer cleared. Accident Property Damage 09/28/ :18: ROCKLAND RD / S WAUKEGAN RD Narrative: Officer responded to the area of route 176 and route 43 for a two vehicle crash. One driver was issued T42216 for failure to reduce speed to avoid an accident. And one driver was issued T42217 for operating uninsured motor vehicle. Formal SR1050 report on file. Assist Parks 09/28/ :14: E SHERIDAN PL Narrative: Complainant advised there was an issue with bolts loose on playground equipment at Artesian Park. Officer made contact with the maintenance department at the Park District for repairs. Ambulance Assist 09/28/ :19: GREEN BAY RD Narrative: Officer responded to the 500 block of Green Bay Rd for a male who suffered an injury. The Lake Bluff and Lake Forest Fire Departments responded along with ambulance A421. The subject was transported to Lake Forest ER. Officer verified with fire personnel no further Police assistance was needed and Officer cleared. Parking Problem 09/28/ :08: E WOODLAND RD / SHERIDAN RD Narrative: Officer responded to East Woodland Avenue and Sheridan Road for a complaint of a vehicle parked too close to the intersection creating a hazard. Officer met with the driver of the vehicle and advised him of the complaint. The vehicle was relocated and the complainant did not want to be seen.

3 Alarm Residential 09/29/ :48: E CENTER AVE Narrative: Officers responded to the 200 block of E Center Ave for an activated burglar alarm. Upon arrival officers found the exterior secured. Dispatch advised no keyholder response. Officers reported no irregularities. Serdar 130 Lost/Mislaid Property 09/29/ :38: E CENTER AVE Narrative: A resident came to the PBS reporting he had lost his wallet, possibly in Roselle or Buffalo Grove, over the past weekend. The wallet contained the subject's driver's license, social security card, and permanent resident card. Officer advised the subject to go to the DMV and Social Security Office and obtain replacement cards, and provided a business card with the case number. Animal Complaint 09/29/ :45: MOFFETT RD / E SHERIDAN RD Narrative: Officer responded to the area of Moffett Rd and E Sheridan Rd for a white dog running loose in the area. Officer checked the area and could not locate the dog. Public Health and Safety 09/29/ :58: SYLVAN RD / GLEN AVE Narrative: Officer responded to Glen Ave and Sylvan Rd for a report of the stairs leading down to the ravine to be broken. Officer found the stairs in good condition. Officer located one board that was broken and a service request was sent to Lake Bluff Public Works via fax. Assist Fire 09/29/ :25: ROCKLAND RD / GREEN BAY RD Narrative: Officer responded to the area of Rockland Road and Green Bay Road for a possible blown transformer. The Lake Bluff Fire Department responded to the scene. ComEd Officials on scene advised the issue was caused by an animal. Officer verified with fire personnel that no additional police assistance was needed cleared. Suspicious Vehicle 09/29/ :53: E SHERIDAN PL Narrative: Officer responded to Artesian Park for a suspicious vehicle. The caller stated a gold in color Ford Explorer with damage to the right side was driving around the area suspiciously. The caller stated a male driver was driving the vehicle and was last seen driving southbound from the area. Officer checked the area and could not locate the vehicle. Solicitor Complaint 09/29/ :59: LAKELAND DR Narrative: Officer responded to the 600 block of Lakeland Dr for a solicitor complaint. The caller stated a male was walking around the area with a clipboard. Officer located the subject and was issued W28820 for Soliciting without a Permit. Officer provided the subject a courtesy ride to the Lake Bluff PSB. Suspicious Person 09/29/ :57: SHERIDAN RD / E WITCHWOOD LN Narrative: Officers responded to Sheridan Rd and E Witchwood Ave for a report of a male wearing a green sweatshirt with a hood over his head and blue jeans. Lake Forest located the subject on the bike path along McKinley Rd south of Park Ln. The subject stated he was staying with family on E Witchwood Ave. Officers observed no irregularities and cleared.

4 911 Call Investigation 09/30/ :59: N SKOKIE HWY Narrative: Officer responded to the 800 block of Skokie Hwy for a 911 investigation. Officer met with an employee who stated she had accidentally dialed 911 while attempting to access her voic . Officer reported no irregularities. Found Property 09/30/ :06: ROCKLAND PLAZA - Narrative: Officer met with a subject at the PSB who stated he found a brown wallet containing an IL drivers license, LINK card, Medicaid card and other miscellaneous cards at the Rockland Plaza. Officer contacted Gurnee Police Department where the owner of the property lives and asked them to make contact with the owner of the property and advise him that his wallet/property is at the Lake Bluff Public Safety Building. - Traffic Stop 09/30/ :12: E CENTER ST / MOFFET ST 09/30/2015 : 17:27:50 rkass Narrative: Officer conducted a traffic stop on East Center Avenue at Moffett Road and issued W28675 for expired registration. Kass #141 Juvenile 09/30/ :57: CAMPBELL CT Narrative: Officers were dispatched to the area of southbound Green Bay Rd at Belle Foret Dr for a juvenile matter. Assist Ambulance 10/01/ :41: S WAUKEGAN RD Narrative: Officer responded to the 100 block of Carriage Park Lane for a subject having a medical issue. Lake Forest Fire Department and Knollwood Fire Department arrived on scene and assumed command. Subject transported to Lake Forest Hospital by Ambulance 421. Officer stood by until properly relieved. Alarm Residential 10/01/ :03: E SHERIDAN RD Narrative: Officers responded to the 100 block of E Sheridan Rd for a burglar alarm. Officers found the exterior secured, and reported no irregularities. Dispatch advised there was no keyholder response. Lock Out 10/01/ :24: ROCKLAND RD Narrative: Officer arrived on scene and met with the subject who stated that he accidently locked his keys in the car. A waiver release form was filled out and signed and officer was able to gain entry into the vehicle. Animal Complaint 10/01/ :28: ARMOUR DR / GREEN BAY RD Narrative: Officer responded to Armour Dr and Green Bay Rd for a report of a black lab running loose. Officer checked the area but could not locate the dog.

5 Accident Property Damage 10/01/ :08: SKOKIE HWY / ROCKLAND RD Narrative: Officer responded to the Lake Bluff Police Department to take a delayed traffic accident report. Officer completed the SR-1050 report and supplied each party with a copy. Animal Found 10/01/ :20: LINCOLN AVE Narrative: Officer responded to the 700 block of Lincoln for a dog at large, the complainant was holding on to the dog. Officer arrived on scene and determined the dog belonged to a resident on Mawman Ave. Officer responded to Mawman Avenue but was unable to reach the owner. Officer transported the dog to Knollwood animal hospital where it was impounded. Suspicious Vehicle 10/02/ :11: GREEN BAY RD Narrative: Officer responded to the area of the 900 block of Green Bay Road for a delayed report of a suspicious older model vehicle pulling into the driveway and turning off its lights. When the refused complainant exited his residence, the vehicle pulled out of the driveway and drove northbound on Green Bay Road. Officer checked the area to the northern limits but was unable to locate the vehicle. No further assistance was needed and officer cleared. Assist Fire 10/02/ :26: E WOODLAND RD Narrative: Officer responded to the 300 block of E Woodland Rd for a downed wire. Lake Bluff Fire Department personnel responded. The wire was determined to be a Comcast cable wire. The wire was removed from the street, and marked with caution tape. Comcast was notified of the situation. No further police assistance was needed. Alarm Residential 10/02/ :03: E SHERIDAN RD Narrative: Officers responded to the 100 block of East Sheridan Road for an activated burglar alarm. Officer arrived on scene and checked the exterior of the building finding eat to be secured. The key holder will not be responding. Officer observed no irregularities and cleared to resume patrol duties. Alarm Residential 10/02/ :11: BLUFF RD Narrative: Officers responded to the 700 block of Bluff Rd for a burglar alarm. Officers made contact with an employee who advised the alarm was accidental. Officers reported no irregularities. Alarm Residential 10/02/ :31: E SCRANTON AVE Narrative: Officers responded to the 300 block of East Scranton Avenue for a residential panic alarm. Officer located an employee, who made contact with a resident inside the house. Officer confirmed that the alarm was set off in error. No other irregularities were observed and Officers cleared. Lock Out 10/02/ :04: E CENTER AVE Narrative: Officer responded to the 600 block of East Center Avenue for a vehicle lock out. Upon arrival, officer met with the vehicle owner and a Waiver/Release Form was completed and signed. Officer gained entry into the vehicle and cleared.

6 Accident Property Damage 10/02/ :35: W SCRANTON AVE / GREEN BAY RD Narrative: Officers responded to the area of Green Bay Road and West Scranton Avenue for a property damage vehicle accident. The driver of unit 1 was subsequently arrested for driving with a revoked driver's license as well as obstructing justice. The driver of unit 1 also had an active warrant out of Lake County for Failure to Appear in Court on a DUI Offense. The subject was transported to Lake County Jail An Illinois Traffic Crash Report form was completed. Found Property 10/02/ :08: E SCRANTON AVE Narrative: Officers responded to the block of East North Avenue in an attempt to locate the owner of the found property. Solicitor Complaint 10/02/ :47: LINCOLN AVE Narrative: Officer responded to the 600 block of Lincoln Avenue for a report of a male subject in a white colored truck going door to door trying to sell meat. The complainant asked for the area to be checked. Officer checked the area and could not locate the vehicle or subject. Alarm Business 10/03/ :12: ROCKLAND RD Narrative: Officer responded to the 900 block of Rockland Road for a report of an activated burglar alarm. Officer arrived on scene and noted no irregularities and found the exterior secure. Officer was advised that no key holder was responding at this time and cleared. Traffic Arrest 10/03/ :14: S RT 41 Narrative: Officer conducted a stop on a vehicle. The driver was issued T42378 for driving while license suspended and the vehicle was released to a valid driver. Suspicious Incident 10/03/ :26: W SANCTUARY LN Narrative: Officers responded to the block of West Sanctuary Lane for a report of an unknown subject attempting to open the front door to the residence. Upon arrival officers checked the exterior of the residence and observed no irregularities. Officers met with the complainant who stated she heard the door being pushed on and someone attempting to open the door using the key pad. Officers checked the area and located a subject sleeping in the driver's seat of a parked vehicle. The subject was taken into custody for underage consumption of alcohol and transported to the PSB for processing. Officers determined that subject was an intoxicated juvenile who attempted to enter the complainant's residence by mistake. Suspicious Incident 10/03/ :32: LINCOLN AVE Narrative: Complainant responded to the Public Safety Building to report a suspicious circumstance. On 10/02/15 at approximately 5:00pm a male subject rang to door bell at her residence located on Lincoln Avenue and stated he was selling meat from his truck. She observed a white colored truck with no logo on the side parked on Lincoln Avenue. She told the male she was not interested and he left the area. She had no other information or description.

7 Citizen Assist 10/03/ :21: SKOKIE HWY Narrative: Officer responded to the 400 block of Skokie Highway for a vin inspection. Officer filled out the documentation Ordinance Violation 10/03/ :30: E NORTH AVE Narrative: Per the request of a complainant who requested to remain anonymous officer responded to the 500 block of E. North Avenue for a possible yard sale ordinance violation. Officer located 4 chairs for sale in the front yard and spoke to the home owner who stated she will obtain a permit and removed the chairs. Public Health and Safety 10/03/ :50: ROCKLAND RD / S WAUKEGAN RD Narrative: Officer responded to northbound Waukegan road and Route 176 for a report of the traffic lights not functioning properly. Officer arrived on scene and checked the light which were functioning normal. Assist Fire 10/03/ :54: E PROSPECT AVE Narrative: Officer responded to the 500 block of E. Prospect Avenue and assisted the home owner shut off the water supply to the home due to a water filter system leaking water. Lake Bluff and Knollwood Fire Departments responded and the scene was turned over to the Lake Bluff Fire Department. No other police service was requested. Harassment 10/03/ :56: BRIAR LN Narrative: Complainant responded to the Public Safety Building for advise on a possible harassment by telephone. Report on file Assist Fire 10/03/ :01: NORTH SHORE DR Narrative: Officer responded to the 900 block of North Shore Drive for an activated fire alarm. Prior to arriving, Officer was advised that the alarm was set off in error. Officer remained on scene until Lake Bluff Fire Department reset the alarm and no police assistance was required. Assist Fire 10/03/ :25: PHILLIP CT Narrative: Officers responded to the 100 block of Philip Court for an activated fire alarm. On scene, it was determined that the alarm was set off by cooking smoke. Officers remained on scene until the Lake Bluff Fire Department required no additional assistance. Check Well Being 10/03/ :36: W BLODGETT AVE Narrative: Officers responded to the 200 block of West Blodgett Avenue for a check well being. The complainant stated they were supposed to meet the residents there and no one was answering the door. Upon arrival, Officers spoke with the residents who stated they just returned home. They will be contacting the complainant.

8 Ordinance Violation 10/03/ :19: LAKELAND DR Narrative: Officers responded to the area north of Lakeland Drive for a fireworks complaint. The caller stated the fireworks were north of her possibly near the ravine. Officers checked the area and were unable to locate any fireworks or irregularities. The complainant did not want to be seen and officers cleared. Alarm Business 10/04/ :04: ROCKLAND RD Narrative: Officers responded to the 900 block of Rockland Road for an activated burglar alarm. Upon arrival officers located an unsecured side door. Officers checked the interior and observed no irregularities. Officers were able to secure the door and were advised no key holder will be responding. 911 Call Investigation 10/04/ :17: MOFFETT RD Narrative: Officer responded to the 100 block of Moffett Road for a 911 investigation. Upon arrival officer met with the homeowner who stated there are no problems at the location. The homeowner stated the call came from the fax machine and he will contact the phone company if the problem continues. Traffic Problem 10/04/ :35: NORTH SHORE DR Narrative: Officer responded to the 900 block of North Shore Drive to meet with an employee regarding people parking their vehicle's on the private property lots and walking to a car show. Complainant did not want an enforcement done, however he requested the officer patrol the area in an attempt to deter parking on private property. Suspicious Incident 10/04/ :08: SUNRISE AVE Narrative: Officers responded to the 500 block Sunrise Avenue for a suspicious incident, front doors facing the lake were open and no one was home. Officer checked the exterior which met with no irregularities. Officer made entry and check the interior of the residence. Officer observed no irregularities inside. It appears that the doors were pushed opened by strong winds. Officers secured the residence and call and left a message for the homeowner. Alarm Business 10/04/ :32: SHERIDAN RD Narrative: Officer responded to the 600 block of Sheridan Rd for an activated burglar alarm. Prior to arrival, officer met with an employee who stated that the alarm was set off on accident. Motorist Assist 10/04/ :19: W WASHINGTON AVE Narrative: Officer responded to the area of W. Washington and Rt 41 for a disabled vehicle. Officer made contact with the driver who stated he had a tow truck enroute. The vehicle was in a parking lot and was not obstructing any traffic. Check Well Being 10/04/ :08: S WAUKEGAN RD Narrative: Officer responded to the parking lot in the 200 block of Waukegan Rd for a well being check. The caller stated a male subject driving a green Ford Escort appeared in need of help. Officer checked the area and located the vehicle, unoccupied. The caller was no longer on scene and officer observed no irregularities. The vehicle registered to an address in Village limits. No assistance was needed and officer cleared.

9 Check Well Being 10/04/ :46: SHERIDAN RD Narrative: Officers responded to the area of Sheridan Road between the Lake bluff Train Depot and the Great Lakes Train Depot for a well being check. The complainant stated that a male subject was sitting on the side of the road and was unable to provide any further description but was concerned for the subjects well being. Officers checked the area of Sheridan Road to our north limits, the Great Lakes Train Depot, the Lake Bluff Train Depot, and the surrounding area side streets and Crab Tree Farms and were unable to locate the subject.
