2 Flight Flight and Cabin and Crew Cabin Fatigue Crew Fatigue Information Virgin Australia Just Culture Policy Virgin Australia is dedicated to providing and maintaining a safe and healthy workplace for team members, contractors, visitors and guests as is practically achievable. Virgin Australia has a 'no blame' policy for the reporting of accidents and incidents. This means that individuals are not punished for actions, omissions or decisions taken by them based on their experience and training which results in a reportable event. The purpose of reporting is to learn from the experience and implement appropriate processes to ensure the safety of team members, contractors, visitors and guests and serviceable equipment. Whenever an accident, incident or personal injury occurs or whenever working conditions, which are potentially hazardous to persons, equipment or buildings are detected, they must be reported and investigated. Policy Virgin Australia Airlines has a responsibility to ensure the safety of all team members, contractors, guests and the general public. Consistent with this responsibility, Virgin Australia Airlines is committed to proactively measuring, mitigating and managing the risks associated with fatigue. Flight and Cabin Crew who experience fatigue or a potentially fatiguing event are encouraged to submit a Flight and Cabin Crew Fatigue. The purpose of these reporting processes are to provide crew with the opportunity to notify of crew fatigue or potential fatigue, thereby providing feedback that can be used to improve the system. In any instance where a crew member has undertaken a fatigue mitigating action directly affecting the operation or conduct of duty, the submission of a Flight and Cabin Crew Fatigue is mandatory. This includes: Removal from planned duty Use of company provided resource (e.g. hotel accommodation, taxi voucher for transport). Submission The Flight and Cabin Crew Fatigue report is used by Flight Crew and Cabin Crew to report instances of fatigue. IMPORTANT: Abuse of this program will result in disciplinary action. Fatigue reports must be submitted within 24 hours of the occurrence, or as soon as reasonably practicable. Only one report is required per duty day, this means that if you are reassigned to other duties and are still fatigued you do not have to complete an additional form. IMPORTANT: ALL reports submitted are in confidence. page 02

Paper-based completed by those team members and contractors with limited access to computers. Figures 1 and 2 illustrate the two different methods of reporting. Operational Personnel s submitted by all operational team members and contractors relating to: International operations including sectors to/from: Abu Dhabi Kuala Lumpur Los Angeles Non-Operational Personnel s submitted by all team members and contractors based in: United Arab Emirates United States Operational Personnel s submitted by team members and contractors relating to: Pacific and Trans-Tasman operations Non-Operational Personnel s submitted by all team members and contractors based in New Zealand and Pacific Islands. There are a number of different types of reports available in the Safety and Security e system. Not all reports are available in all companies. Please contact your Supervisor or company liaison should you require assistance in report selection. page 04

5 Flight Flight and Cabin and Crew Cabin Fatigue Crew Fatigue Flight and Cabin Crew Fatigue To create a new report: 1. Login to Safety ing: VINE > Safety ing > Click the company logo. Note: this report applies to VAA and VAI only. 2. Click Flight and Cabin Crew Fatigue from the list of available reports. Note: the active report is indicated by an orange highlight. 3. Complete the following fields: a. Personal Section: Staff Number Crew Name Crew Type Airline Flight Number Fatiguing Sector(s) Base Have you been removed from flying due to fatigue? Enter your staff number. Enter your first and last name. Choose Pilot or Cabin Crew from the drop down list. Select from the drop down list or enter the airline: Virgin Australia Virgin Australia International Virgin Australia New Zealand Virgin Australia Regional Airlines Alliance. *Specifying the airline will ensure your report is submitted correctly. Enter the Flight Number on which fatigue became a factor. Enter the sector during which fatigue became a factor, e.g SYD BNE. Select from the drop down list your home base. Select Yes or No from the drop down list. page 05

Choose the Aircraft Registration from the drop down list. Enter the date and time of the fatigue event in the format: dd mm yy hh:mm. Select from the drop down list the location where the fatigue event occurred, e.g. Brisbane (not necessarily your home port). Select Yes or No from the drop down list. Occurrence Title Enter a brief description of the occurrence e.g. Flight Crew Fatigue (up to 80 characters). b. Rest or Sleep Information Section: Sleep in 24 hours prior to sign-on Sleep in 48 hours prior to sign-on Hours Awake at sign-on Allocated rest (VAI Only) Hours allocated crew rest (VAI Only) Hours slept in crew rest (VAI Only) Enter the number of hours of sleep in the last 24 hours. Enter the number of hours of sleep in the last 48 hours. Enter how many hours you were awake prior to shift commencement. Pertains to inflight rest facilities. Note: VAI crew members only. Enter the number of hours provided for in-flight crew rest. Note: VAI crew members only. Enter the number of hours of sleep achieved in the crew rest period. Note: VAI crew members only. page 06

Enter your allocated room number. Enter the hotel check in date. d. 10 Flight Flight and Cabin and Crew Cabin Fatigue Crew Fatigue Appendix 1 Fatigue Factors Descriptors Fatigue Factor Description Examples Roster/Pairing Design Factors Time since previous duty Length of duty Layover length Circadian disruption Fatigue that is a result of the time since sign-off from the previous duty to the next sign-on at home base. Fatigue that is a result of the total length of duty time from sign-on to sign-off. Fatigue that is a result of the amount of time allocated for layover sleep (from sign-off to signon) not being long enough to allow for enough rest. Fatigue that result from an inability to achieve adequate sleep due to the rest opportunities occurring at times which are in conflict with the body s circadian rhythm. It is the result of an alteration or interruption of the body s circadian rhythm. For example, going from a series of late duties immediately into early starts, where the crew member has fallen into a pattern of achieving sleep later in the night and has found it difficult to obtain adequate sleep prior to the commencement of the early start. I end up with a late sign-off, had minimum rest at home base and a mid-morning sign-on the following day. I operated a long duty and at the end of the day I felt fatigued. Due to being on minimum rest there was insufficient time to achieve a sufficient amount of sleep or a satisfactory meal. I operated a back of the clock flight and then had to signon at 0600 the following morning. I was only able to achieve one sleep opportunity in a 24 hour window which caused fatigue. Day of Operations Factors Called out Disruption/flight delay Extension of duty Roster change/trip swap Fatigue that is a result of being called out to perform a duty that was not originally rostered. Fatigue that is a result of a duty being disrupted and/or delayed after having signed on for the day. Fatigue that is a result of a duty being extended and requiring discretion to complete above duty limits. Fatigue that is a result of a duty being changed by Crew Control or because the crew member accepted/requested a trip swap with another crew member. I was contacted at 0300 and asked to sign-on at 0500 for a duty on my Designated Day Off (DDO) which I accepted but this has resulted in fatigue. After having completed the first sector of my three sector duty, the aircraft became unserviceable and I had a one hour delay prior to departure for the final two sectors. My duty was delayed after sign-on and I agreed to extend past duty limits; as a result I then became affected by fatigue. 1. I was originally rostered a single sector duty on the first day of my four day trip, however crew control added two additional sectors to my day making it a three sector long duty which resulted in fatigue. 2. I agreed to do a trip swap with another crew member and as a result of the trip I swapped into I have become fatigued. page 10

Fatigue that is a result of not having a crew break during the duty day. Fatigue that is a result of the amount of workload experienced during the day. 1. We were very busy on all sectors and I was not able to have anything to eat. 2. There were no crew meals provided during my duty and therefore I had nothing to eat. Very busy on all sectors and I was not able to have a break. 1. Due to weather issues I experienced additional workload (Flight Crew). 2. Due to full guest loads and service requests I experienced additional workload (Cabin Crew). Inflight Rest Factors (VAI Only) Duration of inflight bunk rest Quality of inflight bunk rest HOTAC Factors Hotel transport time Quality of hotel rest Personal Factors Disrupted home rest Personal health Fatigue that is a result of the amount of time provided for crew bunk rest. Fatigue that is a result of poor quality sleep while in crew bunk rest. Fatigue that is a result of the transfer time from the airport to the hotel. Fatigue that is a result of poor quality sleep at the company supplied hotel while on layover. Fatigue that is a result of sleep being disrupted while at home. Fatigue that is a result of personal health (not illness or injury), such as stress, preventing you from sleeping. Note, an inability to sleep or fatigue as a result of an illness or injury does not fall within the scope of the FRMS. Due to extended service and aircraft issues there was a reduction in bunk rest. Due to turbulence I was unable to achieve any recuperative sleep/rest while on crew rest. 1. After sign-off and there was no transport to be found. After 30 minutes the transport arrived. 2. Due traffic and road works the commute to the hotel that normally takes 20 minutes, took 45 minutes. Due to noise at the hotel I was unable to get good quality sleep as I was woken numerous times. Due to construction works/road works/neighbours or other, sleep at home prior to my duty was constantly interrupted. Due to personal circumstances I have been under a lot of stress lately and have been unable to sleep as a result. Death of a close family friend has resulted in me unable to obtain sleep as I am emotionally drained. page 11
