Information Required Application / Registration Because of the nature of this study-tour, it is important that Biblical Byways receive a considerable amount of information regarding your personal identification, your health, and your travel plans. All of the required forms appear near the end of this Biblical Byways Handbook. II. Application/Registration form should be filled out completely and accompanied by the Deposit and the signed Waiver. If you do not have your passport yet, or if you have not booked your flight yet, then you may send in your registration form without that information. But please send ALL of the information required as soon as you can. III. See the Due date for the following forms: Application & deposit Signed Legal Waiver (with the application) Balance of Payment (due by March 1, 2017) Doctor s Release (due by April 1, 2016) Flight Information (as soon as you book your flights) IV. INSTRUCTIONS: 1. You may apply to Biblical Byways either by or regular post. If by , please answer all questions by computer if possible. 3. Insert an electronic photo of yourself. (If necessary, fill out the form by hand, attach a photo, then SCAN the entire document and it to the coordinator.) 5. You may send a doc file or a PDF file, but DO NOT send a jpg, tif or other picture file. 6. Attach the document(s) to an message and send to BiblicalByways@gmail.com. 7. There should be three pages to each application, plus Doctor s Release and Flight Information. 8. Note that medical and accident insurance is required for all participants. If you do not carry such insurance or cannot find it, ask the coordinator for suggestions. 9. Note that other forms may be required of you, such as a Dr s Release. I have carefully identified, reviewed and considered the risks of travel to my destinations, and I have registered with the Embassy of my home country. 2. I have been provided a description of the proposed program by the agency that is leading the tour, and, if required, I have received approval from my own employer to attend this tour. 3. I have verified that my health insurance is appropriate for my travels. I accept the fact that if I die in Israel without repatriation insurance, my body will be buried there within 24 hours (for about $6000) and not returned to my home country. 4. I hereby release and covenant not to sue Sar-El, Biblical Byways, LLC, Wycliffe Bible Translators, Summer Institute of Linguistics, and their officers from all liability and claims arising out of loss, damage or injury, including death, that I may sustain while traveling to and participating in the Biblical Byways Study-tour. I assume full responsibility for any risks of such loss or personal injury. It is my express intent that this Waiver shall bind the members of my family and spouse if I am alive, and my heirs and personal representative if I am deceased. 5. I know conditions at my destinations may change rapidly. It is my responsibility to stay informed of current events on a frequent basis, either by being informed by our tour guide or by obtaining updated security and health information. TRAVELER S SIGNATURE DATE Transportation: Each participant will be traveling to and from Israel by commercial aircraft, individually contracted. Within the country tour buses (contracted by SarEl) and taxis will be used. Proposed Itinerary is appended: Tour members will stay at the hotels and guest houses as mentioned. The organizers of the tour will make every attempt to identify and avoid unsafe areas. When they do, the itinerary will be changed without prior notice.

5 Doctor s Release Registration Form, p. 4 If you are over 65 years old, you must have a physical exam and your doctor must sign this form. You may send a scanned copy with the doctor s signature by to Biblical Byways via BiblicalByways@gmail.com or a signed hard copy by April 1, Physician s Recommendation: My client, appears to be in satisfactory physical condition to travel to Israel in June, He / She is years old on this date. I understand that the tour will include multiple days of walking quickly on uneven terrain and climbing many stairs at 2500 ft elevation. The walking is usually done in 15-minute increments, but may total up to 5 miles a day. This patient, including his/her heart, lungs, back, knees, and hips, seems healthy enough to participate in such activity for the two-week tour without too much difficulty, (signed) (date)
