Annex A: Vendor registration form must be filled otherwise the applicant will be disqualified Annex B: Required Quantity Specification & Description Annex C: Selection & Evaluation Criteria Annex D: Past Performance and Specific Experience. Applicant Illegibility: The potential vendors must be registered with the government of Afghanistan and have a valid AISA or Business license. A Vendor s current, valid AISA or Ministry of Commerce Business License must be attached with its quotation. The potential vendors should provide an Official Bank Account with the same name as it is stated in Vendor s AISA or business license. The potential vendors should accept tax withholding based in Afghanistan Tax Law. All the cost should be stated in Local currency (AFN).and USD Selection Criteria: The selection will be with best value price and service charges meeting the technical specification listed under scope of work (Attachment B). Vendors should confirm the issuance ticket time (in calendar day) for providing the requested items at due location mentioned in this RFQ after receipt of the purchase order. Selection criterion will only be applied to the quotation(s) that are going to meet the required criteria. Payment: Jhpiego will only make payment in your company maintained bank Account after the delivery and acceptance of the required goods and/or Services by Jhpiego, You are requested to send a sealed and stamp quotation (With RFQ number, and fill attachment A) to Jhpiego Kabul Office below address as stated above by the close of business of the closing date mentioned above. Please remember to state the RFQ Number on the sealed envelope. Envelop without RFQ number will be considered as disqualified. Please attach your company registration, Tax identification number and company bank details with your application.

8 Annex (C) Selection & Evaluation Criteria The award will be made to a responsible offeror whose offer strictly follows the RFQ instructions, meets the eligibility requirements, and meets or exceeds the minimum required specifications of the assignment, and is judged the technically accepted lowest price and best services offer. Technical Competence presented in the Technical Proposal (100 points) The number of points below indicates the relative importance of each individual criterion: No. 1 Criteria Visa Fee, domestic ticket cost and ticket date change 24 hours prior to the departure date. Max Points Domestic ticket No show + refund charges. The No show + date change charges should be fixed price Minimum required points Remark 3 International airfare charges. For international airfares, the ticket cost is not fixed price it depends on the priorities, seat class, seasonality, airline and the destination order Three years related Past performance/experience, Copy of Contracts/Certificates or POs must be submitted Total Maximum Points Best-offer quotations are requested. Award will be made to a responsible supplier who scores the minimum technical points (80) required with the lowest price.
