Panel Discussion SMS Evolution & Current Status Moderator: Panelists: 1500 Tea Break Alex de Silva, President, Alex de Silva & Associates Peter Blake, Senior VP, Safety, Security & Compliance Dept., Korean Air Perry Chou, AVP Corporate Safety Division, China Airlines Len Cormier, Chief Technical Advisor, ICAO COSCAP SEA Norbert Krueger, Vice President for Quality Management, Lufthansa Technik Philippines George Snyder, President & CEO, GHS Aviation Group Douglas Molin, Director, Asia Pacific, MITRE Corp 1530 SESSION 3: RESPONDING TO REGIONAL THREAT Runway Overrun Protection System At the opportunity of the A380 program, Airbus developed a new system to avoid runway overrun. The origin is an improved braking system allowing to vacate at a taxiway designated by the crew. Safety protections are included to avoid runway overrun. First, computations are permanently made in approach to warn if landing is possible on wet or dry runway. Then, when on the runway, if necessary, maximum automatic braking is activated and audio warning asked for maximum reverse selection. This ROPS system will be implemented on all Fly By Wire aircraft in the future
The Challenge of Balancing Safety, the Law, and the Public Interest A discussion of how to maintain the highest level of safety in the face of increasing judicial involvement in accident investigations. It is not the purpose of the safety investigation to apportion blame or liability. On the other hand, the sole purpose of legal proceedings is to apportion blame or liability, thus restricting the free exchange of critical safety information. Which of these is in the best interest of the public? Practical Application of Fatigue Risk Management in Commercial Aviation Joakim Oskarsson, Manager, Professional Services Asia/Pacific, Jeppesen How good are really the flight time limitations in their ability to protecting from crew fatigue? And is there a structured approach of aligning and improving flight time limitations such as FAR, JAR, CCAR and CAP with current sleep science? 1700 End of Day COCKTAIL & GALA DINNER Sponsored by Rizal B Ballroom, Level 2
0800 Registration ASIA PACIFIC AVIATION SAFETY SEMINAR 2010 CABIN SAFETY WORKSHOP Makati AB Room, Level 1 Missman Leham, Assistant General Manager Flight Operations (Flight Safety & Human Factors), Malaysia Airlines 0835 Session 1: Cabin Events Turbulence Management From The Cockpit and Cabin Crew s Perspective Kwok Chan, Head of Corporate Safety, Dragonair Crew and passenger injuries from turbulence encounters have remained the top 2 threats in the past 3 years in the Asia Pacific region. We often believe there is not much we can do to mitigate this risk apart from advising cockpit crews to continuously monitor their weather radar. The presenter will share some of the recent SOP changes in his airline to help reduce the risk of injury from turbulence encounters for crews and passengers. Challenges of Medical Incidents In-flight Heidi Macfarlane, Vice President Strategic Development, MedAire The transportation industry in general and commercial aviation in particular has a peculiar client relationship: duty of care. Medical events regularly occur on airliners and are expected to grow. Sometimes events are serious enough to demand unscheduled landings resulting in airline costs and passenger travel disruptions. This presentation will discuss in-flight medical event trends and solutions. Managing and Responding to Disruptive Outbursts Katharine Ng, Director of Training, OneCrew In this workshop, Katharine will introduce two cabin incidents where the cabin crew were able to successfully manage and respond to the disruptive outbursts. She will then illustrate how these successful stories fit into a simple framework for negotiation skills as a strategy for resolving disruptive passenger situations Session 2: Training/Fatigue Cabin Crew CRM / Training Wu Shou I, Cabin Training Instructor, China Airlines Cabin LOSA Malaysia Airlines The Challenges of Cabin Crew Fatigue Risk Management at Malaysia Airlines Capt. Niazi Saif-Ud Dean Mohammmed Hamid, Flight Operations Manager (Human Factors), Malaysia Airlines Cabin crew fatigue is an understudied, underappreciated, and potentially unsafe issue that needs serious attention. Malaysia Airlines has recognized that the landscape has changed in terms of cabin crew workload and is trying to find a happy medium with the requirement to optimize crew operating patterns Tea Break / End of Cabin Safety Workshop
0800 Registration HUMAN FACTORS WORKSHOP 21Manila AB Room, Level 1 Patrick Murray, Associate Professor, Director Aerospace Strategic Study Centre, Griffith University, Australia 0835 Session 1: Cockpit Operations Cockpit Technology Change Steven Khoo, Manager Flight Operations Safety & Security, Singapore Airlines From the pioneering days of the Comet to the present day A380, Singapore Airlines has experienced permanent change from expansion; contraction; changes to existing systems, products and services; and introduction of new equipment or procedures. Hazards will be introduced and safety management practices require that these hazards be identified, implemented and subsequently evaluated. Stall Recovery Procedures These last years, several fatal accidents happened, on various aircraft types, due to inappropriate stall recovery technique. Discussions between aircraft Manufacturers led to change the recovery procedure. Instead of trying to minimize altitude loss, the procedure requests now to reduce immediately the angle of attack as soon as the stall or approach to stall is recognized. Stall recovery training is presently being reviewed by all the Manufacturers and the FAA and there will be some recommendations to improve this training. Procedures and training aspects will be covered by this presentation. Evidence Based Training Patrick Murray, Associate Professor, Director Aerospace Strategic Study Centre, Griffith University, Australia Progress in the design and reliability of modern aircraft, together with a rapidly changing operational environment and the recognition that not enough has been done to address human factors issues, has prompted a strategic industry review of airline pilot training. An analysis of the most relevant threats, through evidence collected in flight operations and training can drive regulatory change and enable the worldwide implementation of more effective training standards to improve operational safety Session 2: Training/Fatigue Application of Just Culture the Air New Zealand Experience Tahlia Fisher, Senior Safety Specialist, Airline Operations and Safety, Air New Zealand Whilst Air New Zealand now embraces the widely-accepted philosophies of a Just Culture, employee and managerial understanding of it, and adjustment to its implementation, requires oversight. This presentation demonstrates the application of Just Culture principles within at Air New Zealand and discusses the broader implications of operating in an industry with varying levels of support for Just Culture. Tech Crew CRM / Training Glen Eastlake, Non-technical Skills Manager, Virgin Blue The presentation will examine the Virgin Blue Non-Technical Skills program from an 'organisation accident prevention strategy' perspective. It will define some terms used such as human factors, CRM and non-technical skills and will introduce the concept of system resilience and NTS training. It will outline the types of courses covered at Virgin Blue Airlines and the challenges facing aviation and non-technical skills training. Maintenance Crew Fatigue Risk Management Bill Rankin, Technical Fellow, Maintenance Human Factors, Boeing This presentation will discuss the challenges that the industry faces in developing a Fatigue Risk Management System (FRMS) for maintenance and provide background information on EASA, Transport Canada, and FAA FRMS approaches. An outline of an FRMS will be discussed, and existing FRMS materials will be highlighted Tea Break / End of Human Factors Workshop
0800 Registration ASIA PACIFIC AVIATION SAFETY SEMINAR 2010 SAFETY TOOLS WORKSHOP 21Rizal A Ballroom, Level 2 Tzvetomir Blajev, Senior Team Leader, EUROCONTROL 0835 Session 1: Tools Maintenance LOSA Bill Rankin, Technical Fellow, Maintenance Human Factors, Boeing The U.S. Air Transport Association (ATA) has developed a Maintenance Line Operations Safety Audit (LOSA) process, which was modeled on pilot LOSA, that is now available for deployment. This presentation will discuss the Maintenance LOSA process, including the Maintenance LOSA observation forms, Maintenance LOSA training, and data analysis. FSF Updated ALAR Tool Kit The updated ALAR tool kit has all the original ALAR information, plus updated ALAR data. It also contains results of a recent Foundation project addressing the challenge of runway excursions. Excursions are by far the most common type of aviation accident. Data will be presented that highlight the high risk areas of runway excursions, and interventions for pilots, ATC, airports, and regulators are presented to reduce that risk. Flight Data Monitoring A Service Provider s Perspective Dave Jesse, CEO, Flight Data Services Flight Data Monitoring (FDM) is a tool for identifying flight safety precursors. This presentation will explain how to manage safety using FDM and then examine how to select the right system for your operation. Case studies will be used to show how safety issues have been identified and resolved Session 2: Data Management Sharing Data to Better Manage Safety Risk Len Cormier, Chief Technical Advisor, ICAO COSCAP SEA ICAO has introduced the requirement for Safety Management Systems (SMS) in virtually all aspects of aviation. In addition, ICAO is presently establishing a requirement for a State CAA to have in place a State Safety Programme (SSP). The presentation will provide a brief overview of the linkages between an air operator SMS and the CAA SSP. The presentation will also highlight the importance of sharing data on a national and global basis. SMS Key Performance Indicators GHS/PAL/AAPA Stakeholder Collaboration / Information Sharing Douglas Molin, Director, Asia Pacific, MITRE Corp 1500 Tea Break / End of Safety Tools Workshop
