I've enclosed your Tour Agreement and other important information about your trip which is explained below. As always, if you have any questions, feel free to contact me. Registration Forms Health Records Tour Agreement Baggage Tags As the Tour Organizer, you re responsible for collecting and returning completed and signed forms from all tour participants, including all chaperones, to Gerber Tours. Only ORIGINAL double-sided forms will be accepted for Registration. If the participant is a minor, the form must be signed by a parent or guardian. We recommend collecting Health History and Medical Release forms from everyone in your group. Bring them along on your trip in case of emergency. For your convenience, we have enclosed a sample form. After careful review, please sign and return both copies along with the Registration Forms and the non-refundable booking deposit by the date specified in the Tour Agreement. We will then send back to you a counter signed copy for your records. We have enclosed a pre-addressed envelope for your convenience. Payments and Conditions, Group Agrees to the Following: To provide its own Chaperones to supervise the conduct and welfare of the students. To have all payments and Rooming Lists to Gerber Tours, Inc. as specified in the Payments & Deadlines Schedule on the following page. To submit a completed Registration Form for every Tour participant to Gerber Tours, Inc. 4. Late payments will incur a $100 fee. Payments received within 30 days of departure will incur an additional $36 to cover late processing fees. 5. Cancellation of this tour prior to 60 days of departure will receive full refund, except for supplier contractual fees and charges and the non-refundable booking deposit as specified in the Payments & Deadlines Schedule on the following page. 6. Cancellations occurring within days of departure will be subject to cancellation fees of 20% of the Tour Cost, the nonrefundable booking deposit as specified in the Payment Schedule on the following page and any amounts paid to suppliers to confirm Group's reservations. 7. Cancellations occurring within 14 days of departure will receive no refunds. 8. To the Terms and Conditions attached hereto as Exhibit "A". 9. Gerber Tours will require participants and/or their legal guardians to agree to its terms and conditions attached hereto as Exhibit "B" as well as its other conditions. Binding Arbitration: Any controversy or claim arising out of or relating in any way to these Terms and Conditions, to this Tour Agreement, to the brochure, or any other information relating in any way to the trip, or to the trip itself, shall be settled solely and exclusively by binding arbitration in New York City, New York in accordance with the rules of the American Arbitration Association then existent. In any such arbitration the substantive (but not procedural) law of New York will apply. The arbitrator and not any federal, state, or local court or agency shall have exclusive authority to resolve any dispute relating to the interpretation, applicability, enforceability, conscionability, or formation of this contract, including but not limited to any claim that all or any part of this contract is void or voidable. Join your Tour Guide and transfer to your motorcoach. Visit Rockefeller Center and St. Patrick's Cathedral. Walk and window shop along Fifth Avenue, one of the most popular shopping streets in the world, and visit such icons as Saks Fifth Avenue, Trump Tower, Tiffany & Co and the Apple Store. Time for lunch at your expense. 2:00 pm Admission to the Metropolitan Museum of Art. Visit and tour the Theatre District including Broadway and Times Square. Tour of Upper Manhattan including Cathedral of St. John the Divine, Riverside Church, Grant's Tomb and Harlem. Tour of Central Park including Strawberry Fields, and the Alice in Wonderland sculpture. Time for lunch at your expense. 1:00 pm Admission to the American Museum of Natural History. 5:00 pm Depart for LaGuardia Airport. 8:25 pm Depart LaGuardia Airport via Southwest Airlines Flight # :00 pm Arrive Hartsfield-Jackson Atlanta International Airport. This itinerary is designed to remain flexible to compensate for traffic, crowds and weather conditions.

8 *** Important Information for Air Travelers*** Please make note of the following items regarding ticketing and traveling by air: Tickets Names on all tickets must be the Participant s full, legal name exactly as it appears on the Participant s ID that they will present at the airport. Nicknames or shortened forms of names may not be used. For this reason, please make sure that the names on all Registration Forms are full, legal names exactly as it appears on their ID so that we have accurate information for ticketing. Passengers with incorrect names on their tickets may not be permitted to board their plane. Once Gerber Tours tickets the group, any name change from names submitted on Registration Forms is subject to a $150 fee. Fares Airfares are not guaranteed until ticketed and are subject to price adjustments as a result of tax changes, fuel surcharges and/or airport fees. Baggage Many airlines are now charging fees to check baggage. Please see attached Baggage Notice for more details. Identification Domestic Flights: All passengers 18 and older are required to present valid and unexpired government-issued photo identification (federal, state or local). For example: driver s license or passport. Name on ID and ticket must be an exact match. Children under the age of 18 traveling with an adult are not required to show ID at time of check-in. However, we recommend all passengers carry some form of identification and children close to 18 years of age should also carry proof of age (ie. birth certificate). Flights to/from Canada: All U.S. citizens including children must present a valid U.S. passport. Name on passport and ticket must be an exact match. Check-In Airlines require groups to arrive at the airport at least 1 ½ hours prior to departure.

9 IMPORTANT AIRLINE BAGGAGE NOTICE: As you are probably aware, many airlines are now charging fees for checked bags. Due to the constantly changing nature of these fees and the difficulty in pre-paying them, these fees are not included in the price of your airfare. As a result, many clients have advised their trip participants to limit their luggage to carry-on items only. Most airlines allow one carryon bag plus a personal item such as a purse, briefcase or small backpack. Should your group wish to check baggage, please see the attached chart for current pricing and where to obtain the most up to date information.
