The aircraft burst into flames as the fuel tanks ruptured on impact, destroying the house of Douglas and Karen Wielinski, and most of the plane. Douglas was killed; his wife Karen and their daughter Jill managed to escape with minor injuries. 2 On February 2,, the NTSB issued its final report, describing the details of its investigation that led to 46 specific conclusions. Among those conclusions were the fact that both the Captain and the first officer responded to the stall warning in a manner contrary to their training. The NTSB could not explain why the first officer retracted the flaps and suggested that the landing gear should also be retracted, though it did find that the current approach-stall training was not adequate: The current air carrier approach-to-stall training did not fully prepare the flight crew for an unexpected stall in the Q and did not address the actions that are needed to recover from a fully developed stall. Hart and Board Member Robert L. Sumwalt, III dissented on the inclusion of fatigue as a contributing factor, on the grounds that there was not enough evidence to support such a conclusion. It was noted that the same kind of pilot errors and standard operating procedure violations had been found in other accidents where fatigue was not a factor. One of the most significant changes has already taken effect, changing the way examiners grade checkrides in flight simulators during stalls. The NTSB theorized that due to this low tolerance in a tested simulation environment, pilots may have come to fear loss of altitude in a stall and thus focused primarily on preventing such a loss, even to the detriment of recovering from the stall itself. New standards subsequently issued by the FAA eliminate any specific altitude loss stipulation, calling instead for "minimal loss of elevation" in a stall. One examiner has told an aviation magazine that he is not allowed to fail any applicant for losing altitude in a simulated stall, so long as the pilot is able to regain the original altitude. Tenerife disaster[ edit ] The Tenerife airport disaster, which occurred on March 27,, remains the accident with the highest number of airliner passenger fatalities. There were no survivors from the KLM aircraft; 61 of the passengers and crew on the Pan Am aircraft survived. Pilot error was the primary cause, as the KLM captain began his takeoff run without obtaining air traffic control clearance. The KLM flight crew could not see the Pan Am aircraft on the runway until immediately before the collision. An increased emphasis was placed on using standardized phraseology in air traffic control ATC communication by both controllers and pilots alike. The captain is no longer considered infallible, and combined crew input is encouraged during aircraft operations. The crash of Japan Airlines Flight on August 12,, is the single-aircraft disaster with the highest number of fatalities: The aircraft suffered an explosive decompression from an incorrectly repaired aft pressure bulkhead, which failed in mid flight, destroying most of its vertical stabilizer and severing all of the hydraulic lines, making the virtually uncontrollable. All 15 crew members and of the passengers on board died. The death toll was exacerbated by delays in the rescue operation. Although a number of people survived the impact, by the time the Japanese rescue teams arrived at the crash site all but four had succumbed to their injuries. The collision was mainly the result of the Kazakh pilot flying lower than the assigned clearance altitude. All passengers and crew on board of both the aircraft died. The crew were all Malaysians, while the passengers were of various nationalities, most from the Netherlands. According to a Dutch report, high-energy objects hit the plane in midair, causing it to break apart. According to the preliminary report of the international investigation commission, the plane was shot down by Russian occupation troops using a surface-to-air missile fired from the Ukrainian territory that they occupied. 4 and crew aboard the aircraft died. The official report says bad weather brought the aircraft down; high winds and fog were present at the time of the crash. This broke hydraulic lines, causing leading edge lift devices to retract on that side of the aircraft and resulted in asymmetrical lift and loss of control. The accident was attributed to improper maintenance procedures. The crash resulted in the deaths of all passengers and crew on board, as well as two people on the ground. All passengers and 16 crew, and 11 people on the ground all residents of Sherwood Crescent, Lockerbie, died, [24] [25] making it the worst terrorist attack involving an aircraft in the UK and the deadliest terrorist attack on British soil. Following the crash, the Federal Aviation Administration imposed new security measures on American airlines flying out of airports in Western Europe and the Middle East. All people on board, as well as five people on the ground, died from the crash. All passengers and 14 crew members were killed. It is the deadliest aviation accident involving a DC-8, the largest aviation disaster involving a Canadian-registered aircraft and the second-worst accident in Saudi Arabia. On April 11,, an Algerian Air Force Il transport plane crashed shortly after take-off from Boufarik Airport, killing all passengers and 10 crew on board. On December 12,, a Douglas DC-8, Arrow Air Flight, carrying American military personnel on a charter flight home for Christmas, crashed in Newfoundland ; all passengers and 8 crew members died. It carried 12 Malaysian crew members and passengers from 15 nations. Numerous theories have been offered to explain the disappearance of the flight, but none has been confirmed. On September 26,, an Airbus AB, Garuda Indonesia Flight, which departed from Jakarta, Indonesia, and was preparing to land at Medan, North Sumatra, crashed into mountainous terrain, killing passengers and 12 crew members. The causes included turning left instead of right as instructed by the ATC and descending below the assigned altitude of 2, feet due to pilot error. Kennedy International Airport on a flight to Paris and Rome. A lengthy investigation concluded that the probable cause of the accident was a short circuit in a fuel tank that contained an explosive mixture of fuel vapor and air. As a result, new requirements were developed to prevent future fuel tank explosions in aircraft. The crew reacted incorrectly, leading to an aerodynamic stall from which the jet did not recover. On August 6,, a Boeing B5, Korean Air Flight, crashed on approach to the international airport in the United States territory of Guam, killing of the people aboard. On January 8,, an Antonov AnB aircraft with 6 crew members on board overshot the runway at Kahemba Airport and plowed into a market place. Four on board survived but people on the ground were killed and an estimated were injured estimated seriously injured. It is usually known as the Air Africa crash. It was determined that the crash, which killed all passengers and 19 crewmembers aboard the plane, was caused by improper repairs to the aircraft 22 years earlier when the aircraft encountered a tailstrike. A branch of the Islamic State of Iraq and the Levant claimed responsibility for bringing down the jet, and a Russian investigation concluded that a bomb was detonated inside the plane at a high altitude. The National Transportation Safety Board determined that the probable cause of the crash was deliberate action by the relief first officer in response to his removal from international service within Egyptair, a finding disputed by Egyptian authorities who maintain another cause of the accident. An investigation concluded that the captain became disoriented after the failure of one of the flight instruments in the cockpit, leading to "irrational control inputs" that caused the plane to crash. As the aircraft approached the airport, the pilot executed a sudden steep ascent that produced a stall and crash. China Airlines was also criticized for "insufficient training. All passengers and 9 crew were killed. An investigation concluded that the plane went down due to pilot error. The air crew used an inappropriately low airspeed, causing vibrations that they incorrectly interpreted as engine surges. It was the most deadly transportation accident in the country since February, said National Transportation Safety Board chairman Robert Sumwalt. He added the investigation is focused to find "not only what happened, but why it happened so we can keep things like this from happening in the future. The accident also raises new questions about limousine safety. Three years ago, four women were killed in a limousine on Long Island as they toured wineries when the vehicle was hit by a truck. A driver and the front-seat passenger are required to wear seat belts inside stretch limousines, but not the back-seat passengers, police said Sunday. After the Long Island crash, Sen. Chuck Schumer said he urged the NTSB to better investigate limousine accidents in the country, saying Sunday, "because we desperately need more data to prevent them and minimize their toll.
