The aircraft called Wilson Air Traffic Control (ATC) Tower for taxi and departure clearances. The aircraft had two persons on board and five hours of fuel endurance. The aircraft took off from runway 14 at 0414 hours. It was seen by ATC to rotate shortly before the intersection of the two runways with the initial climb lengthwise the runway before a right turn was made. During the climb, the pilot reported to ATC that he wanted to land immediately on runway 32. At this time, the succeeding aircraft on runway 14 was already lined up and it was advised to immediately leave the runway in order to give way to the accident aircraft. At 0416 hours, the aircraft crashed short of runway 32 at approximately 200 meters from the threshold and on a heading of approximately

4 The probable cause of this accident was the pilot s decision to turn back and land on the same runway from which he had just taken off. 5 The incident was reported by Kenya Airways personnel at about 0430 hours and on site investigation commenced at about 0530 hours. The aircraft was arriving on a scheduled passenger flight from Amsterdam. It was allocated Apron 1 Stand number 2. There was a Boeing 747 (B744) already parked at the same apron on Stand number 3. As the aircraft taxied to its stand, the left wing struck the right winglet of the B744. The cause of the collision was determined to be the allocation of the two aircraft to adjacent bays whose dimensions could not accommodate the two aircraft at the same time. Contributing to the incident was the lack of recurrent training for the marshalling staff. 5

The aircraft had been chartered by a mining company based in Mwanza and was to drop two company employees in Migori. One of the employees was picked at Mwanza while the second was to be picked in Nairobi. During landing the aircraft touched down normally and during the ground roll the nose landing gear collapsed. The propellers struck the ground as the pilot struggled to stop the aircraft. The aircraft stopped at the intersection of runway 14 and 07 blocking both the runways. On raising the nose and dismantling the nose gear door, we found the nose gear fractured in multiple locations. Investigation could not determine the cause of the gear failure but the fractures were considered to have been as a result of the incident. 6

Light damage on wings The aircraft took off from Eldoret International Airport and was airborne at 0443 hrs UTC. The destination was Djibouti. Takeoff was from runway 08. It was reported that the aircraft was seen to stop at least two times during taxi before takeoff. After takeoff the aircraft made a left turn and commenced to set course for Lokichoggio. The pilot reported that the engine thereafter, started to lose power but did not shut down. As the engine lost power, he realized that they would not be able to fly back to the airport as they were losing altitude at the same time. The aircraft made a left turn to avoid the power 7

8 lines that were in their flight path and almost immediately hit the ground in a flat attitude. It bounced over some uneven ground and the outboard starboard wing tip including the tip tank was torn from the right wing. The aircraft slid onto the wheat farm ahead and settled facing the airport to the South. Residents from the adjacent built up area rushed to the scene and tried to help the occupants who suffered various degree of injury. They were rushed to hospital where they were admitted. The aircraft had been fuelled with normal automobile fuel (mogas). The aircraft, type Piper PA-34 was operated by Auric Air of Tanzania. The pilot was given clearance by Air Traffic Control to land on runway 07. As he made his landing, eye witnesses observed that he touched down just before the intersection of runway 07 and 14 and immediately applied brakes. The controller confirmed that smoke was coming from the wheels as the pilot applied the brakes. On observation of the left main landing gear tyre showed 8

9 marks of heavy braking. As the aircraft rolled down the remaining runway the pilot realised that he would not be able to stop on the runway and opted to veer to the right to avoid coming into contact with the perimeter fence. He then decided to steer his aircraft back onto the runway and attempt to make a 180 turn to backtrack the runway. In doing so the aircraft side slipped on the right main landing gear and broke the gear. The aircraft stopped on the runway at right angles with it. Investigation revealed that the pilot had touched down too far down the runway and therefore had inadequate runway length within which to stop. DHC Operator Aircraft type 5Y-STN ALS Ltd. During the take off roll and at a speed of approximately 87 knots the flight crew heard a loud 9

10 bang followed by aircraft instability. The aircraft seemed to be having a right turning tendency. As the right hand turning tendency increased the crew opted to reject the takeoff as the airspeed was below V1. They used reverse thrust as well as left brakes which were still available. As they slowed down the cabin attendant reported to have seen smoke from the tyres, followed by the smell of burning tyres. The aircraft finally came to a rest to the right of the runway after application of emergency brakes. Both the engines were shut down before the aircraft came to a complete stop. The two right main landing gear tyres blew during the incident. There was no other damage or injury. An inspection of the remaining tyres indicated that they were serviceable. Airbus A VT-EJI Aircraft type Airbus A Airbus Industries Operator Air India VT-EJI Serial number 413 Type of engine General Electric CF6-80C2A2 Date of accident 28 th August 2006 Time of accident 1520 Hrs Location of accident End of runway 06 JKIA Nairobi Nature of flight Public Transport Persons on board 140 Injuries Nil All tyres burst during rejected take-off The flight, AI-200, aircraft type Airbus A , was due to take-off on a flight from Jomo Kenyatta Airport Nairobi to Mumbai. The crew requested and were given ATC clearance to start the engines and to proceed on the flight level 330. ATC departure clearance was given at 1509 hrs which was duly read back by the crew. The aircraft lined up at 1521 hrs and were cleared to take off at 1523 hrs. The crew duly read back the clearance. The aircraft commenced the take-off roll 10

11 and as it reached V1 speed, lost power on the number 1 engine. The crew opted to abort the take-off, advising ATC about the aborted take-off at 1524 hrs. They advised tower that they would be exiting the runway via the right, and proceeded to exit via taxiway L. After vacating the runway the aircraft stopped with the tail just inside the runway area. Thereafter, the aircraft could not be moved because all eight tyres had burst and the brakes were binding from the heat generated during the emergency stop. This led to a partial closure of the runway. On board the aircraft were the flight instructor and a student pilot. The aircraft took off from runway 07 at 0939 hrs and headed for a climb through flight level 63 when the pilot noticed decay in the rate of climb enroute to the local training area. The pilot initiated the emergency procedures before crash landing at the Jamhuri grounds. 11

12 The aircraft crash landed on a rough field and stopped 9 meters from the point of first impact. The instructor and his student were then rushed to the hospital with minor injuries. The aircraft was substantially damaged as a result of the crash. An engine run in the shop indicated that the engine compression was low in a number of cylinders and was considered to have contributed to the low power generated by the engine during the accident flight. Cessna 208B 5Y-NPD Aircraft type Cessna 208B Cessna Aircraft Corporation 5Y - NPD Aircraft serial number 208B0369 Date of registration 12 TH April 2005 Type of engine PT6A-114A Date of incident 23 rd October 2006 Time of incident 0900 hrs Location of incident Between Lokichoggio and Bunagok South Sudan Type of flight Public Transport Number of persons on board Crew 2 Passengers 1 Injuries Crew None; Passengers None Aircraft substantially damaged Captain s flying experience 6500 hours (of which 900 were on type) The aircraft was on a flight from Lokichoggio Airport in Kenya to Bunagok in South Sudan. Approximately 1.5 hours into the flight, the engine started leaking oil which spilled onto the windscreen of the aircraft, imparing the pilots vision. There was smoke in the cockpit and the pilot declared an emergency. The pilot broadcast their position and then prepared for an emergency landing. The aircraft made a successful emergency landing in an open field nearby. After ascertaining that there would be no fire, the pilot radioed his company from the 12

13 aircraft and help was sent to evacuate them. They were airlifted by helicopter at about 0940 hrs. A strip investigation in an overhaul shop did not reveal any significant failure to the engine. LET 410/Cessna 208B 5Y-BRU/5Y-SLA Aircraft type LET 410-E9 & C208B LET Company & Cessna Aircraft Corporation 5Y-BRU & 5Y - SLA Aircraft serial number LET & C208B-0574 Type of engine Walter M601 & Pratt & Whitney PT6A-114A Date of incident 24 th October 2006 Time of incident 0900 hrs Location of incident Kichwa Tembo Airstrip Masai Mara Type of flight Commercial Number of persons on board Crew 2 & 1 respectively Passengers 3 & 9 respectively Injuries Crew None; Passengers None Slight wing tip damage on both aircraft Pilot s flying experience 31 hours (of which 6 were on type) The LET 410 aircraft had dropped and picked passengers at Kichwa Tembo airstrip and was taxiing out when the pilot broadcast her intention to take off as is the procedure. At this time the aircraft was lined up at the end of runway 08 ready for takeoff. The Cessna Caravan aircraft was coming in for a landing from Musiara airstrip and about to line up with the runway for the landing. The pilot of the Cessna aircraft responded to the other pilot indicating his position and that he was lining up with the runway. The LET 410 pilot realized that it was not possible to take off before the other aircraft landed. She however believed that there was adequate space for the landing aircraft to make the landing and to vacate the runway. She did not vacate the runway, as would have been expected but remained parked at the end of the runway. The landing aircraft made a successful landing on runway 26 and as it was vacating the runway, the Cessna pilot released the brakes and started to roll for her take off. There was inadequate space for the two aircraft as positioned and the wing tips touched. 13

The aircraft, Sierra Leone registration 9L LFQ, type Antonov AN - 12, was operated by 748 Air Services Limited. The aircraft departed Jomo Kenyatta International Airport at 1212 hours and arrived Lokichoggio at 1344 hours. On landing the aircraft bounced several times before the nose wheel collapsed. The aircraft came to a stop about 100 meters from the threshold of runway 27. The crew evacuated safely without any injuries. Efforts to clear the runway started immediately. Several light aircraft landed before the runway was cleared. Two aircraft were diverted to Juba. The runway was cleared by 0100 hours and normal flight operations resumed by 0455 hours. The probable cause of the accident was a higher rate of descent than prescribed during landing leading to bouncing. After the first touchdown, the aircraft bounced three times. This, and the fact that the aircraft was at high loading, lead to the failure of the nose gear. 14

The aircraft, type Cessna P206E, was operated by Blue Nile East Africa Limited. The aircraft departed Wilson Airport at 1159 hours and arrived at Mandera at 1535 hours. It was already dark at the time of arrival and Mandera Airstrip has no lighting. The Army Unit stationed at Mandera positioned two lorries to assist the pilot to land the aircraft. The aircraft touched down well but during the landing roll it veered off the runway and ended up in the ditch about 150 meters from the end of the runway. The pilot had minor injuries while the other two occupants were not injured. This incident was caused by the pilot s poor flight planning and his decision to land by night on an aerodrome that did not have lighting. 15

On board were the pilot and two passengers. At about 100 feet above ground level, the pilot initiated a shallow bank to starboard in the direction away from the high ground. During this maneuver, the aircraft stalled and impacted the ground. There were no injuries and no fire. The aircraft received substantial damage. The probable cause of this accident was the pilot s decision to take off on runway 08/26 towards higher ground instead of runway 18/36. Unfavorable winds around the higher ground were a contributory factor. 16

The aircraft veered off the runway to the right of the runway centerline. There had reportedly been rain in the recent past and the runway and the ground around was wet. Efforts to recover the aircraft through application of differential power were unsuccessful. The weather at the time of accident was good with excellent visibility with no wind. The aircraft stopped off the runway next to an embankment and suffered damage to the right propeller and the right main landing gear. The Comoros authorities attended to the incident but did not provide a final report. 17

21 SYNOPSIS On Saturday, 30 th December 2006, at about 0328 hours, a Buffalo DHC-5, registration 5Y-SRK, operating as flight number RED448A, crashed after takeoff from runway 06 at Jomo Kenyatta International Airport (JKIA). This was an International Committee of the Red Cross (ICRC) cargo flight to Bardera in Somalia. The flight was conducted in accordance with an Instrument Flight Rules (IFR) flight plan. On board were the captain, first officer and a technician, all employees of Sky Relief. The aircraft was destroyed by impact forces and subsequent fire. All 3 occupants of the aircraft received minor injuries. The probable cause of the accident was the failure of the left engine, for undetermined reasons, on take off. The bad weather experienced that day was a contributory factor. 21
