Shortly after the departure from CYQX, the number 2 engine went into an uncommanded feather. The flight crew shut the number 2 engine down in accordance with the QRH, declared an emergency and, since the weather at CYQX was at minimums, diverted to Deer Lake (CYDF), NL. The flight landed at CYDF without further incident. An analysis of the uncommanded auto-feather of the number 2 engine determined that it was caused by a loss of the number 2 engine torque signal. The related torque signal sensing/transmitting components have been sent to the manufacturer for further investigation.

Approximately 20 nm North of CYQX, the flight crew received a Right Engine Bleed Line failure warning. The flight crew executed the check list and switched the right bleed air valve off. A PAN PAN was declared, and the flight landed at CYQX without further incident. The operator s maintenance determined that a faulty bleed air warning temperature sensor caused the false indication.

At approximately 7000 feet ASL, one of the skydiving passengers climbed out of the right hand side of the aircraft and up onto the right wing. Shortly thereafter, the second skydiving passenger climbed out onto the right hand landing gear to film the first passenger. The passenger on the right wing laid down parallel to the direction of flight, and began to move slowly toward the tip of the wing. Suddenly, the aircraft entered a spiral dive, with the right wing low. The pilot then motioned to the second passenger to release from the aircraft's landing gear, while the aircraft continued to spiral to the right. C-GAGP, a Cessna 310Q aircraft registered to Eagle Eye Aviation, was filming the stunt and declared an emergency when the pilot noticed that C-FZMD was in the spiral dive. After an undetermined amount of revolutions to the right, the pilot of C- FZMD regained control of the aircraft at approximately 1500 feet AGL. It is unknown when the passenger on the top of the wing departed the wing surface; his parachute failed to open and he suffered fatal injuries. The second skydiving passenger deployed his parachute and landed without further incident. The aircraft returned to Vernon, BC (CYVK) and landed without further incident. The RH Aileron and the top wing surface were found damaged. The RCMP is investigating.

Pearson Intl (CYYZ), ON. When the flight crew selected Flaps 15 during the approach to Runway 23 at CYYZ, they received a TE FLAP DISAGREE EICAS message. The approach was discontinued, and the flight crew executed QRH procedures. The fault did not clear and, consequently, a PAN PAN was declared. The aircraft landed uneventfully with ARFF standing by. The operator s maintenance determined the #1 RVDT was out of limits and subsequently proceeded to replace it. An operational check of the system revealed no further faults, and the aircraft was returned to service.

During the landing roll out, the aircraft went off the left side of the runway. The main wheels dug into soft mud, causing the aircraft to nose down into the mud, and settled back on its landing gear. There were no injuries, however the aircraft sustained substantial damage.

The aircraft was substantially damaged, and the pilot sustained serious injuries.

During cruise flight at FL320 approximately 760 nm West of EGLL, the aircraft experienced what was initially believed to be severe or wake turbulence for approximately 2 seconds. It was later determined that there was no altitude or speed loss, and the event was rather classified as moderate wake turbulence. There were no injuries to the occupants, or damage to the aircraft. The aircraft continued to its destination for an uneventful landing.

Pearson Intl (CYYZ), ON to Thunder Bay (CYQT), ON. When the flight crew selected landing gear down during the final approach to Runway 25 at CYQT, the nosegear doors did not open and, consequently, the nose landing gear did not extend. The amber nose-gear door light did not illuminate, and the red nose-gear unsafe light remained illuminated. Additionally, the landing gear handle also remained illuminated. The flight crew conducted a goaround, and requested ARFF to stand by for the landing. QRH items were executed, and an alternate landing gear release and extension was performed. The subsequent landing on Runway 25 was successful, and the flight crew brought the aircraft to a full stop on the runway with ARFF responding. Gear pins were installed, and the aircraft was towed to the gate. The operator's maintenance replaced the Proximity Sensor Electronic Unit (PSEU), and the aircraft was returned to service.
